Description
Being the Bilingual Licensed Case Manager you will be responsible for the implementation, and coordination of trauma informed case management services to clients in the Emergency Housing Voucher Program (EHV). Using a client centered, and trauma informed approach, the Licensed Case Manager coordinates survivors full-cycle care., The Licensed Case Manager will follow-up with clients and partners to continue to build collaborative relationship with internal and external partners to best meet the survivors physical, emotional, and spiritual needs. The Licensed Case Manager will employee a HAWC approved case management process that promotes wellness., The survivor lead approach consists of advocacy, communication, education, identification of service, access to resources, and facilitation of service delivery to achieve long term housing and EVH grant outcomes.

- Monday Friday 9am- 6pm
- Work Model: hybrid
- Local Travel:50% (or as needed)
Our Total Compensation Package:
- Starting Salary Range: $27.89 hourly
- Pay schedule bi-monthly

Qualifications
The Requirements We are Seeking:
- Masters in Social Work and LMSW licensure in good standing
- Five years case management experience
- Fluent in Spanish: able to read, write, and speak fluently
- Intermediate proficiency within Microsoft Office specifically Excel and Outlook.
- Texas drivers license with a good driving record; ability to travel to off-site locations utilizing personal transportation, if applicable
What you Deliver in this role:
- You will assess domestic and sexual violence survivors who have chronic or acute head and psychological trauma impacts that require it.
- You will ensure safety, housing stability and trauma recovery of survivors served in the EHV program
- You will collect qualitative and quantitative data to support successful EHV outcomes
- You will ensure housing program survivors are safe(r), stable and thrive using a collaborative model
- You will build rapport and relationship with survivors being served through reflective listening, trauma education and support
- You will help to break down barriers, mobile advocacy is required. This includes meeting the client in the community at home, work, or other safe places, etc. and accompany clients to appointments as needed
- You will implement services with a trauma-informed, evidence-based, and client-centered approach while assisting clients working toward EHV outcomes
- You will use Motivational Interviewing, positive reinforcement, reflective listening, harm reduction and other evidence-based intervention strategies to help the survivor identify and address behaviors that put the survivor at risk for losing housing
- You will provide transportation and transportation options as needed to assist survivor achieve their goals on the empowerment plan.
- You will review and are knowledgeable of EHV program regulations and data standards.
- You will ensure timely and accurate data entry of all needed metrics for reporting required as well as timesheets.
- You will ensure unit eligibility, income verification and other required documentation is collected when needed and assistance to client to obtain documentation is provided in advance to ensure deadlines are met.
- You will document and communicate accomplishments and challenges; Attend trainings and meetings
