Community Health Worker (HCH)
(Full Time; 40 hours/week)

Generations Family Health Center, Inc. is a regional community health center covering a thirty-seven-town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers.

LOCATION: Willimantic, CT

JOB SUMMARY: Under the direction of a designated supervisor/ manager, the Community Health Worker screens individuals or families using a standardized tool. The Community Health Worker is responsible for providing individuals with relevant and program specific educational materials. The Community Health Worker provides individuals and families with community referrals as needed. They will be responsible for marketing the Healthcare to the Homeless program to the community and attending community collaboration meetings, workshops and events. The Community Health Worker may also assist with the development and ongoing continuity of collaborative relationships with community sites and programs. Coordinates designated administrative functions of community-based/ health care programs to ensure smooth facilitation of patient/client flow, participation and high-quality customer service. Provides community health outreach, health education and other support services to targeted populations to improve awareness about designated programs and resources. The Community Health Worker may be required to work offsite in support of the Healthcare to the Homeless program where applicable.

  • Strong organizational skills to handle varied duties and responsibilities.
  • Strong public speaking skills with the ability to talk in front of groups.
  • Basic computer skills including Word and Excel.
  • Ability to conduct groups and educational workshops as needed.
  • Understands complex family issues and dynamics and be sensitive to families.

Qualifications

Associate’s degree in human services or related field, or equivalent related course work and work experience desired. Minimum of two (2) years experience with logistics coordination and outreach/case management, preferably in a health care setting. Valid driver’s license and availability to properly insured vehicle throughout employment. Bilingual candidate (Spanish/English) preferred based on patient population.

Pay Rate: $18.00/ hour
