The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The Colorado Coalition for the Homeless seeks an Case Manager I to provide short term patient-centered case management services for recuperative care clients. These services include providing help with housing, public benefits, medical/mental health/substance treatment referrals, obtaining identifying documents and other needs with the goal of ending their homelessness. This position also develops working relationships within and outside of the organization to improve client access to needed services and maintains documentation of all services provided. This position is located at 2175 California St, Denver, Colorado 80205 and reports to the Health Services Case Management and Peer Service Program Manager.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate’s submission of documents necessary for clearance.Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
- Develops comprehensive individual care plans and goals for the client with input from the other Recuperative Care Team members and the client.
- Obtains appropriate community resource information and makes referrals as indicated.
- Maintains ongoing contact with client during recuperative care stay.
- Provides and documents case management activities according to agency policy. These activities may include (but are not limited to) transporting clients, providing advocacy within the community, making community referrals, assist with housing acquisition, life skills building, identification and benefit acquisition.
- Participates in all planned meetings with Recuperative Care team, coordinated intakes and discharge visits, and collaborative weekly patient check-ins when applicable.
- Engages with patient’s medical and clinical stakeholders as needed.
- Provides services to clients in a trauma-informed, culturally competent manner.
- Becomes informed about CCH programs and services.
- Assists with consistent implementation of program policies.
- Builds relationships with clients with the goal of housing, benefits and other resources needed to end clients’ homelessness.
- Has a minimum of 80 client encounters each month with proper documentation in EHR.
- Updates spreadsheets and other internal and external data sources as need for data collection.
- Record all services in a timely and complete fashion in the HER.
- Maintains up to date records in EHR.
- Records telephone and other communications as needed in the EHR.
- Manages tasks and scanning as needed to have a complete and current chart.
- Maintains accurate information regarding client’s insurance status.
- Utilizes the EHR and any future upgrades as directed.
- Answers or promptly responds via cell phone to urgent needs when working offsite or absent.
- Meet required COVID-19 vaccination(s), as defined by applicable law, regulation, or guidance from the Centers for Medicare and Medicaid Services.

Qualifications Summary
- Bachelor’s degree in a human services or related field preferred. Combination of related experience and education may substitute for degree.
- Experience, competence, and sensitivity in working with people experiencing homelessness, especially those who have difficulty engaging in treatment, suffer from mental illness, or substance addition or are otherwise impaired.
