Description

Position Summary: A Child Support Professional works as part of a team in coordinating the daily operations of the Community Residence Program and ultimately ensures the safety of the youth served. A Child Support Professional reports to the Program Supervisor/Social Work and the Program Director.

Essential Accountabilities:

1. General supervision of youth during shift. All youth must be in eyesight of all staff at all times.

2. Provide hand-off communication with other staff members any time a youth enters or leaves an area of eyesight vision.

3. Ensure and adhere to OMH regulated 3:1 daytime/evening and 4:1 overnight youth to staff ratios at all times.

4. Primary Counselor for 2-3 youth.

5. Complete Goal Review Updates bi-weekly for primary youth as assigned. Complete crisis plans every 3 months.

6. Contacting assigned youth’s families multiple times per week.

7. Complete monthly restorative notes as assigned, complete progress notes as assigned, and complete collateral contact notes for every contact with families, doctors, dentists, psychiatrists, therapists, schools, outside agencies and any other contact regarding the youth.

8. Daily entries in general log.

9. Completing the shift summary sheets as assigned.

10. Documentation of the youth phone contact with their families.

11. Transportation of youth in agency and/or personal vehicle. Accompany, supervise and provide transportation (when necessary) for appointments, home visits and activities.

12. Involvement in change of shift meeting and house youth meeting.

13. Attendance at all in-service trainings and OMH trainings.

14. Attendance at 75% staff meetings annually (9 of 12 staff meeting) and playing an active role in bringing up staff issues and youth concerns.

15. Medication management and appropriate documentation.

16. Implementation of service plan goals and objectives, assisting youth in learning appropriate daily living skills, and assistance in all activities (i.e. meal preparations, housekeeping, and recreational activities, etc).

17. Emergency coverage as needed (i.e. staff shortage on shift, crisis CPEP visits after scheduled hours).

18. Ensure proper change of shift (i.e. not leaving until the next shift arrives).

19. Report to Program Supervisor or Program Director for bi-weekly supervision.

20. Participation in OMH Q1 plans and agency Q1 plans.

21. Swiping in at the beginning of shift and swiping out at end of the shift for all shifts worked including trainings at other WellLife Network sites. Submission of timesheets on the Thursday of the pay period ending.

22. Responsible for maintaining (or having access to) a personal vehicle throughout the course of the work day.

23. Submission of car insurance policy before the expiration date to supervisor.

24. Attendance and participation at the Family Bonds Retreat Weekend, if necessary.

25. Other duties as assigned.

Qualifications

Associates degree in social work or related field. Bachelor’s degree is strongly preferred or a high school diploma and a minimum of two years’ experience in a mental health or related setting.

Valid and clean NYS driver’s license and access to a vehicle.
