Overview

The SPECIALTY HOSPITAL FOR CHILDREN with Profound and Severe Disabilities, located at Terence Cardinal Cooke Health Care Center will be expanding our services in the near future and we are adding staff to this unique unit!

Responsible for the planning and development of policies and procedures with regard to the the functioning of the MCH Social Work Department. Provides direct supervision of the Social Work Staff and consultation to all other staff of MCH. Participates in various Administrative Commitees. Conducts inservice training programs. Maintains contacts with Community Agencies and keeps abreast of change and innovations in social welfare policy and tochniques to constantly enrich and enhance the services provided by the Department.

Responsibilities

  • Leadership of CCP meetings and Coordination of CCP meeting schedule. Coordination of CCP meeting schedule with Family/Guardians. Coordinates meetings between residents/families/guardians and members of the interdisciplinary team and/or administration as needed. Coordinates and facilitates requests for discharge and transfers. Organizes bereavement counseling for residents/families/guardians as needed. Facilitates communication with the interdisciplinary Team on behalf of residents/families/guardians. Coordinates and facilitates the referral of residents for needed/requested services and resources not available at MCH.
  • Act as resident liaison with the DOE, OPWDD, and DOH. Coordinate meetings between representatives of those agencies and residents/families/guardians as needed. Reporter of incidents/deaths to OPWDD and Justice Center as needed. Represent MCH at meetings/conferences with Outside Agencies as needed.
  • Conducts admissions assessments and interviews with residents/families/guardians to evaluate the appropriateness of placement and to identify the need for special services.
  • Advocates for residents’ physical, mental, and psychosocial well-being. Coordinates and monitors the available services for the resident to ensure the optimal level of physical, mental, and psychosocial well-being.
  • Responsible for supervising the implementation of each individual’s individual program plan, integrating the various services received by each individual, recording each individual’s progress, and initiating periodic review of each individual program plan as stipulated by this regulation. This task will be a minimum of 5 hours per week
  • Coordinate with Finance to monitor residents account balances. Coordinate the spending of resident funds for clothing needs and other quality of life improvements.

Salary Range: $102,538-$115,315 based off education level and/or years of experience in the field

Qualifications

Experience Requirements

  • 2 years of social work leadership experience
  • Office for People With Developmental Disabilities (OPWDD) experience preferred
  • Strong Social Work clinical knowledge, in addition to strong customer service background.

License Requirements

  • Licensed Clinical or Licensed Master Social Worker

Education Requirements

  • Master of Social Work degree from an accredited university.
