What will you be doing in your role?
- You are proactive in your identification of barriers to client success.
- You are informed about community resources, maintain current knowledge of practices and issues around poverty and family self-sufficiency and share with your team.
- You help your team apply a problem-solving framework that promotes steady client progress.
- You support the data and evaluative needs of Padua by ensuring proper documentation and data input is completed.
- You incorporate use of all available resources, drawing on Padua internal and external partnerships to help clients progress with a holistic service plan.
- You work directly with clients on a triad with two case managers.
- You participate in staff training and development.
- You facilitate client connection to necessary resources by providing transportation if necessary.
- You maintain a flexible schedule to occasionally meet with clients after normal business hours.
- You monitor program due dates to ensure your client/team meets all required targets.
- You regularly meet with your case manager to discuss progress towards client goals and ensure that a clear path is available to achieve them.
- You help clients connect with valuable resources by advocating with community partners and by providing transportation as needed.
- You incorporate use of all available resources, drawing on Padua internal partnerships to help clients progress with a holistic service plan
Are you the next Padua Case Worker?
- Bachelor’s degree in social work preferred
- Work experience with low-income populations.
- Familiarity with Salesforce, community resources is preferred.
- Must have reliable transportation and be willing to transport clients as needed.
