Description of Primary Purpose
Primary Purpose: Performs and coordinates social services and case management services, including informal counseling with individuals and families, providing information and referral, accessing people to needed services, developing programming, and supporting the objectives of the Senior Center.
Supervision Received: Work under general supervision of the Manager of Senior Services. Additional case management direction provided by Police Command and Police Social Worker as necessary for continuum of care and emergency care needs.
Full-Time IMRF Pension eligible position.

Essential Duties and Responsibilities
Essential Duties and Responsibilities:
Provides continuum of care services in conjunction and consultation with the Police Department Social Worker and other regional mental health care partners including but not limited to scheduled home visits and emergency response activities as appropriate during normal business hours.
Contributes to Senior Services Division services including front desk coverage, telephone answering, registration, and customer service. Provides activity leadership and assistance including room set-up and break-down, supply organization, and excursion leadership.
Provides emergency response activities in conjunction with the Police Department as appropriate outside of normal business hours; may include nights, weekends, and holidays.
Responds to information and referral inquiries from Senior Center members (age 50+), family, residents, other service providers and City of Highland Park staff.
Oversees and coordinates front counter volunteers; provides management of social services related programs including SHIP, volunteer snow shoveling, and volunteer grass cutting services; coordinates the Medical Loan Closet program.
Develops and facilitates appropriate support groups as part of Senior Center programming. Provides short-term counseling in person on site and via telephone or other electronic means and navigation to ongoing services.
Performs other related duties as requested.
Education and Experience
Education: Master’s degree in counseling, social work, gerontology or related area. Continuing education certifications in crisis and trauma management or other relevant areas favorable.
Experience: A minimum of two years working with older adults or demonstrated interest in working with persons 50+ in social services field. Familiarity working with clients experiencing self-neglect, hoarding, dementia, chronic & acute conditions, and home neglect scenarios preferred.
Certification or License: Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) designation required. Valid driver’s license required.
Required Knowledge, Skills, and Proficiencies: This position requires excellent communication skills and the ability to effectively present ideas both orally and in writing, the ability to produce reports of a varying nature on a timely basis, skills in the operation of a computer, the ability to listen to and give advice and assistance to employees, Senior Center members, as well as the general public, have knowledge and understanding of aging in the older adult/senior population and the various resources available, and the ability to establish and maintain an effective working relationship with City officials, employees, professional contacts and the general public.
Supplemental Information
Work Environment: Most work activities are conducted in an office environment and include significant public contact, sometimes involving individuals exhibiting high levels of stress or emotion. Some activities will take place on site at individual homes and care facilities. The position requires some work outside of normal business hours related to Senior Center activities or in response to emergency care situations including nights, weekends, and holidays.
This employee is occasionally required to perform job duties out of doors, i.e. conducting outdoor programming, assisting members on Center grounds, and day excursions.
Tools and Equipment Used: Office equipment including a computer with Windows and Microsoft Office Suite with a secure server for client case files, copier, printer, scanner, fax machine, cell phone, and desk phone as well as shared passenger car. Emergency/Crisis intervention situations may require additional personal safety/branded clothing or equipment.
Physical Demands: While performing the duties of this job, the employee is frequently required to sit, talk, hear, read computer monitor and printed and handwritten documents and forms; use fingers to operate keyboard to enter information into computer records. The employee must occasionally lift and/or move objects up to 50 pounds.
