National Health Foundation is looking for a Social Services Coordinator at our Recuperative Care facility in Ventura, CA. This is a great opportunity to provide assistance and guidance to individuals who are experiencing homelessness. Our next Social Service Coordinator will bring their knowledge and experience, while delivering these services to our guests in a compassionate and empathetic manner. We aren’t just looking for someone to check off the boxes, we are looking for an individual that is passionate and committed to our mission while believing in the radical hospitality that is being provided to the homeless community.
Pertinent Information
The schedule for this position is Sunday-Thursday 8:00AM – 4:30P. This is an on-site position in Ventura, CA.
The salary range for the position is $50k-$60k/year, and is an hourly, non-exempt role. In accordance with National Health Foundation’s equitable pay practices, NHF would stay within budget for this position.
Per NHF policy, all staff must be fully vaccinated and have received their booster shot when eligible with the exception of medical/religious exemptions
What does success look like as a Social Service Coordinator?
- Provide Individualized Intensive Case Management Services to all of our guests; Complete assessment and care plans for our guests at our facility
- Work with our nursing team to develop a complete aftercare plan for guest discharge from our facility that continues the improvement of their overall health
- Identify housing opportunities for our guests, collaborating with community partners and landlords to establish housing options, and facilitate the discharge process after a housing solution has been identified.
- Coordinate services for mental health while guests stay with us, and set guests up for services after discharge.
- Complete referrals, progress notes and discharge summaries, maintaining an up-to-date database and hospital contacts utilizing the Care Management Platform known as CHAMP. Attend and collaborate during care coordination meetings.
- Create a healthy and positive environment that invites individual growth for our facility.
Requirements
What would you bring to the organization?
- Bachelor’s Degree Required, Masters in Social Work or related field is preferred.
- Minimum of three (3) years relevant experience in healthcare delivery, hospital education, social services, or similar field preferred.
- Ability to communicate effectively, and bring a positive, upbeat, and professional attitude to our team.
- Excellent time management and strong organizational skills.
- Ability to speak a second language that is most commonly used in Ventura (Preferred but not required)
