Description
Position Summary:
The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.
Essential Job Functions:
- Provides case management services and assistance to program participants struggling with substance abuse, and those in recovery;
- Assesses the needs, conditions, and desires of assigned program participants;
- Educates and refers program participants to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc.
- Assists participants with establishing or re-establishing community support systems;
- Acts as advocate on behalf of program participants to improve access to vital services;
- Participates in clinical staffing decisions regarding the participants current strengths and resources, and their need for additional resources;
- Completes documentation of services provided in a timely manner;
- Provides input into the patients treatment needs and disposition;
- Conducts assessments of program participants and provides therapy as needed;
- Reviews and documents all clinical services and activities;
- Participates in case conferences involving assigned program participants;
- Prepares Discharge Summaries, as needed;
- Assists other WestCare staff as needed to promote cooperation and portray a professional image to the community;
- Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation;
- Performs any other duties as assigned.
Qualifications
Certifications/Licensures:
- CADC or similar certification preferred; Preference will be given certified candidates;
- Valid drivers license and the ability to obtain and maintain WestCares vehicle insurance is required.
Education:
- Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred;
- High School Diploma or equivalent is required;
Experience and Competencies:
- Experience working with individuals suffering from mental health and substance abuse disorders required;
- At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred;
- Ability to define problems, collect data, establish facts, and draw valid conclusions required;
- Excellent communication skills are required;
- Ability to exercise good judgment and discretion required;
- Ability to work well in a team environment required ;
- Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required;
- Professional appearance and demeanor required;
- Must be culturally/linguistically sensitive to populations served;
- Intake and assessment experience is highly preferred;
- Bilingual ability is preferred;
- Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired;
Working Conditions:
- Work is primarily performed in an office or emergency shelter setting;
- Some outdoor activities and travel between multiple locations will be required;
- Regular and prompt attendance is required throughout employment.
