2 shifts 7p to 7am & 2- 11-7a per week. Every other weekend

The role of the LPN Nurse Supervisor is to coordinate and assist with the provision of Nursing Care to Residents and to oversee the delivery of care by assigned Personal Care Assistants in accordance to the policies and procedures along with the Mission and RISE Core Values of Fellowship Community and DPW regulations.

The LPN Nurse Supervisor functions under the direction of the Resident Care Director and has a coordinating relationship with the Vice President of Personal Care Services.

ESSENTIAL QUALIFICATIONS / CREDENTIALS

§ Must have graduated from licensed nursing school with an active PA LPN license.

§ Must have knowledge of DHS regulations related to Personal Care.

§ Must have certificate in Basic Life Support.
• Must have computer skills at an intermediate level.
• Ability to read, write, speak and comprehend English.

§ Knowledge of the special needs and care requirements of geriatric residents.

§ Able to effectively present information in one-on-one and small group situations to customers and other employees of the organization.

§ Works well with others in a positive and cooperative manner.

§ Willing to be flexible to meet the needs of the organization.

§ Personal Care Home Medication Training, CPR and First Aide qualified

§ Must demonstrate effective leadership skills in delivery and coordination of resident care.

§ Must have knowledge of special needs and care requirements of Geriatric Residents.

§ Work performance reflects the essential elements of Fellowship Community’s RISE Core Value.

§ Successful completion of Fellowship Community orientation.

§ Meets the requirements of the Pre-hire Assessment.

§ All Fellowship Community employees must be fully vaccinated against influenza and COVID 19. Fellowship Community may provide an exemption to any eligible employee with a qualifying medical condition that contraindicates receipt of the vaccine or on the basis of a sincerely held religious belief and practice.

ESSENTIAL DUTIES AND RESPONSIBILITIES OF RESIDENT CARE

§ Must consult with residents, families, and physicians in providing resident care, treatment, rehabilitation and discharge.

§ Can adequately complete correct and comprehensive resident forms as required by DPW Regulations and under the direction of the Director of Resident Care and the Vice President of Personal Care Services. To include, but not limited to the following: Durable Medical Evaluations, Resident Assessment and Support Plans, fall risk assessments, Pre admission screenings, etc.

§ Administer injections such as IM meds. Or any subsequent medications not in the PCA Team Leader’s guidelines set forth by DPW med. administration classifications.

§ Supervises and directs the delivery of care by the PCA team members designated to their floor.

§ Provides input into the performance reviews of the PCA team members.

§ Support dietary staff at resident meals and report any changes in resident’s meal completion to physician/ Resident Care Director.

§ Receive report at the beginning of shift as well as giving report at the end of shift inclusive of narcotic count and ensuring accuracy of count.

§ Review residents chart for specific treatment, medication orders, diets and review proper order of charts.

§ Consult with physicians for medical and/or prescription needs of residents in the Terrace and write appropriate orders and proper transcription of same.

§ Report any changes of residents medical, psychological or mobility needs to the Resident Care Director, Vice President of Personal Care or the Physician when appropriate.

§ Serves as the Administrator Designee as directed by the Administrator or, Resident Care Director
