Description

GENERAL FUNCTION: This position provides targeted case management, fosters relationships in the community, coordinates partnerships with local schools, and organizes and facilitates individual and group activities and services for youth ages 12-18, including non-minor dependents, and their children in a licensed residential treatment setting. Coordinates services and ensures appropriate documentation for youth in accordance with agency policies.

QUALIFICATIONS:

  • Must be able to be associated with Casa de Amparo through Community Care Licensing (CCL), which typically includes clearance by FBI, DOJ and CACI.
  • Education and/or experience requirements:
    • Have a Bachelor’s Degree (in human science field preferred) OR
    • Have a valid Child Development Teaching Permit OR
    • Have completed 12 semester units of Early Childhood Education, Adolescent Development or Foster and Kinship Care Education and have at least 100 hours of experience working with youth OR
    • Have a valid certificate as an Alcohol Counselor, Drug Counselor or Alcohol and Drug Counselor and have at least 100 hours of experience working with youth OR
    • Have a valid vocational training certificate, credential or documentation demonstrating that the individual is a trade journeyperson who instructs children in vocational skills and have at least 100 hours of experience working with youth as a mentor, athletic coach, teacher, vocational coach, tutor, counselor or other relevant experience as determined by the department OR
    • Have previously been employed as a full time staff or served as a volunteer at a group home, STRTP or substance abuse treatment program for at least one year OR
    • Have relevant life experience in the child welfare, mental health or juvenile justice systems as a consumer, mentor or caregiver or other relevant experience as determined by the department.
  • Must have at least one-year fulltime (or equivalent) experience working with children or youth.
  • Knowledge of developmental needs of children ages 0-18 years throughout the developmental stages preferred.
  • Must possess strong interpersonal, time-management, organizational, written/verbal communication skills, and intermediate computer literacy. English proficient/Bilingual Spanish preferred.
  • Must have ambulatory, auditory and visual skills to observe residents and staff, including the physical ability to participate in activities and perform related physical skills, including, but not limited to, crouching, lifting 50 pounds, crawling on hands and knees, and walking up and down stairs.
  • Experience with Cerner electronic health record preferred.
  • Ability to comply with all employment requirements including, but not limited to CPR/Pediatric 1st Aid, physical, TB and drug screen clearance, clear DMV record, proof of insurance and approval from insurance carrier to drive agency vehicles. Must provide proof of immunization against influenza, pertussis, and measles.
  • Must be at least 21 years of age.

Major Tasks

MAJOR TASKS:

  1. Establish and maintain consistent contact with client’s Placing Agency Representative, authorized representative, HHSA social worker regarding client’s progress, placement planning, medical needs, and outside services.
  2. Assist clients in obtaining personal documents and personal/medical documents.
  3. Complete required documentation, including but not limited to Needs and Services Plans, Quarterly Reports, daily notes, ILS reports and intake/discharge inventories.
  4. May assist in completing intake documentation including but not limited to Residential Services Agreement, Emergency Release forms, Personal Rights, Emergency Intervention Plan, Runaway/AWOL Plan, Violence Prevention Policy.
  5. Ensure professional and timely coordination of services with school district representatives in the community.
  6. Enroll residents in school including sending registration packets to school districts at intake and coordinate transportation for residents.
  7. Coordinate education and education related services for residents.
  8. Educate residents on Personal Rights, House Rules, and Grievance Procedures.
  9. Provide case management that is knowledgeable and respectful of differing cultures and the norms that exist within various cultures.
  10. Ensure CFT referrals are submitted for residents as necessary and appropriate.
  11. Attend and complete documentation for meetings for assigned residents (i.e. IEP, CFT, etc.), as appropriate.
  12. Maintain contract and agency productivity and documentation standards and timelines, documenting clinical services within 72 hours from date of service and maintaining 50% billing productivity.
  13. Work under the direction of a licensed or waivered staff member.
  14. Assist in coordinating activities with program management.
  15. Provide crisis intervention and assistance, ensuring the safety of residents.
  16. Ensures completion of admission and transition personal belonging documentation in accordance with agency policies.
  17. Assist with on-site visits from Placing Agency Representatives.
  18. Assist with facilitating and/or transporting to offsite groups, activities, school, visits, passes, and/or appointments using agency and/or personal vehicle, as necessary.
  19. Provide scheduled care and supervision in the cottages, as appropriate.
  20. Attend all staff meetings and in-service trainings as required.
  21. Perform other tasks as assigned.

EFFECT ON END RESULT: This position assures that targeted case management is provided in a trauma-informed and culturally respectful manner to support the care, treatment, and services of residents and their child/ren in Residential Services.
