Clinician- Young Adult Serivces

POSITION SUMMARY: Support the goals of BHcare by providing clinical services based on best practices to individuals in the Young Adult Services (YAS) residential program, outpatient, intensive outpatient clinic, while collaborating with psychiatrist and treatment team in diagnosis and treatment plan.

Accountable and responsible for the following results:

  • Serve as primary clinician for caseload.
  • Provide intensive individual, group and family psychotherapy to both residential and outpatient clients which is flexible and responsive to clients’ needs and consistent with evidenced-based protocols.
  • Assess clients’ abilities and mental health status, needs and preferences and ongoing appropriateness of services.
  • Assist clients on caseload with developing and implementing fully integrated individual recovery plans that are completed according to agency policy and are based off of the transtheoretical model of behavior change (stages of change) and are person centered. Coordinate comprehensive (integrated) service planning to include members of clients’ treatment team and clients’ natural supports, including YAS and family members, conduct regular reviews of integrated recovery plans and modify as needed focusing on goal resolution and discharge.
  • Provide clinical support, clinical group, and other structured activities at the Young Adult Services Residential site for the residential clients as clinically indicated and based on program needs as determined by the YAS Program Manager and YAS Residential Program Manager.
  • Provide emergency assessment and crisis intervention for both YAS outpatient and YAS residential services. Communicate client needs with hospital emergency rooms and emergency personnel.
  • Conduct intake assessments/psychosocial evaluations and develop initial recovery plans.
  • Demonstrate advanced Integrated Dual Disorders Treatment competencies.
  • Monitor and analyze utilization review and productivity of caseload; make adjustments as needed.
  • Obtain insurance authorization for clients as required and complete all requests for re-authorizations by deadline.
  • Complete progress notes, documenting progress on service goals and objectives, psychosocial histories, admissions and discharge summaries, and other documentation within the Organization’s guidelines for timeliness.
  • Participate in weekly Young Adult Services team meetings for the YAS outpatient program and YAS Residential program. Will also participate in the monthly YAS THP meetings.
  • Meet with Young Adult Services Program Manager to review clients deemed to be at risk and pre-contemplative about treatment.
  • Provide at least one clinical contact per week with all clients enrolled on caseload, including mobile involvement when deemed appropriate.
  • Provide coverage support for YAS Clinical team as deemed necessary.
  • Provide timely documentation and data to Organization’s departments, DMHAS, or other sources as required.
  • Perform supervised urine screening and Breathalyzer test as needed.
  • Collaborate with hospital staff regarding inpatient treatment and discharge planning.
  • Serve as a liaison between client, family, other Organization programs, other parts of the mental health system, and other community resources. Provide advocacy as appropriate.
  • Inform and refer clients for benefits for which they may be eligible. Complete clinical documentation for benefits as needed.

Qualifications

MINMIMUM QUALIFICATIONS

  • Master’s or doctoral degree in social work or other related field required.
  • Clinical experience in a psychiatric or substance abuse setting preferred.
  • Must be proficient with PCs and Microsoft Office Word & Outlook, able to perform concurrent documentation and navigate through electronic health record.

PROFESSIONAL LICENSURE /CERTIFICATIONS

  • Active CT Lic -LMSW or LCSW preferred. LPC, LMFT, and LADC may be considered.
