I. POSITION GOAL

Provide field-based clinical case management, counseling, and victim advocacy to survivors of elder and dependent adult abuse.
II. PRIMARY RESPONSIBILITIES

Holistic Elder Abuse Response Team (HEART) and APS Center of Excellence

  1. Provide clinical field-based case management to elder and dependent adult abuse survivors throughout Los Angeles County.
  2. Conduct announced and unannounced visits to client homes as part of the provision of service to clients.
  3. Provide individual and group counseling to elder and dependent adult abuse survivors.
  4. Receive and respond to elder/dependent adult abuse referrals from Adult Protective Services, Police Departments, and other referral sources.
  5. Conduct in-home biopsychosocial assessments of elder/dependent adult abuse survivors.
  6. Provide care coordination and advocacy for elder/dependent adult survivors of abuse, including crisis abatement, safety planning, provision of basic needs such as nutrition and housing, and linkages to additional necessary services.
  7. Collaborate with clients to develop short and long-term care plans using a strengths-based approach.
  8. Accompany and, if necessary, arrange transportation for elder abuse survivors to criminal justice offices and court proceedings.
  9. Assist elder abuse survivors in completing paperwork for restraining orders and attend hearings as necessary.
  10. Assist survivors in filing for victim compensation.
  11. Provide outreach and education about elder and dependent adult abuse to consumers and service providers in the community.
  12. Participate in case discussions at weekly multidisciplinary meetings with legal, social service, and health professionals, including law enforcement and protective services.
  13. Knowledge of city and county agencies that are part of the elder abuse intervention network and secondary agencies that can support elder abuse survivors.
  14. Collaborate with local agencies to provide enhanced services to elder crime survivors.
  15. Attend agency and other meetings as required.
  16. Maintain tracking of program activities and submit monthly, quarterly, and annual reporting for internal and external reporting.

III. SECONDARY RESPONSIBILITIES

  1. Provide wraparound services to elder abuse survivors placed in Adult Protective Services shelter beds.
  2. Provide wraparound services to elder abuse survivors placed in long-term care facilities.
  3. May provide supervision to social worker interns.

IV. ESSENTIAL JOB FUNCTIONS

  1. Ability to provide clinical field-based geriatric case management, therapy, and legal system advocacy to elder/dependent adult abuse survivors.
  2. Ability to provide individual counseling to elder and dependent adult abuse survivors.

III. POSITION QUALIFCATINS

A. Education Requirements

Master’s degree in social work or marriage and family therapy preferred. May also consider a master’s degree in gerontology, human services, public health, or behavioral science

B. Experience Requirements

  1. At least two years of experience in social work, gerontology, or related fields.
  2. Professional experience providing case management and advocacy.
  3. Professional experience in working with victims and aging adults.
  4. Knowledge of, experience with, and ability to assess law enforcement and victim assistance programs.

C. Other Requirements

  1. Strong interpersonal, verbal, and written communication skills.
  2. Working knowledge of Windows, Word, and Outlook.
  3. Ability to interface/relate well to the public, all levels of staff, and other professionals.
  4. Reliable transportation.

IV. CONSTRAINTS

This position is funded by the County of Los Angeles. Termination of all or part of these funds at any time during the funding period may affect the agency’s continued ability to fund this position.

V. WORK ENVIRONMENT

The employee may enter premises during field visits that may be environmentally challenging, including but not limited to environments with cigarette smoke, bed bugs, dust, and other allergens.

This position is housed at the Long Beach Police Department and the Long Beach office of WISE & Healthy Aging. Frequent travel throughout Los Angeles County is required.

Employee must ensure that field offices are secure when leaving the premises.

The employee may be expected to maintain office equipment.
