General Purpose:
Facilitate and complete a wide range of client-based healthcare services.
What I’m Responsible For (Essential Functions):
The following statements regarding this Non-Exempt level position consist of duties, responsibilities, and frequency describing the general nature and level of work performed by the employee filling this position. These statements are not an exhaustive list of all duties and responsibilities required by this position. • Accurate and timely completion of all client data and record keeping based upon input from various sources, including but not limited to, client medication management, general first aid, and nursing support.
• Effectively navigates client records to access information necessary to schedule medical appointments and/or complete provider paperwork.
• Manage clients medical care with providers (scheduling routine care appointments such as physicals, eye exams and dentist visits. Non-routine care appointments such as illness, injury, imaging, and urgent care visits. Coordinating trips to medical specialists as needed, including those outside of Laramie.
• Researching and obtaining medical history and records on clients prior to CHC placement.
• Assist in obtaining access to COVID, Meningitis and HPV vaccines.
• Partner with clients and their families surrounding scheduled treatments, therapies, and hospitalization, (including preoperative/postoperative appointments and directives as well as obtaining guardian consents)
• Understand and comply with HIPAA regulations and maintain strict confidentiality of clients and staff both on and off CHC locations.
• Recognize and comply with all security and safety requirements of the department, to include notifying on-call staff when necessary.
• Perform and assist with client physical holds/restraints as needed for client safety.
• Understand and apply “Six Rights” of the clients in the selfadministration of medication program.
• Maintain the highest level of professionalism and boundaries with clients, families, staff, and providers.
• Demonstrate understanding and application of individual client needs, abilities, and feelings including both developmental and cultural needs.
• Aware of the emotions and behaviors of trauma associated with abuse, neglect, and trauma.
• Professionally communicate with all staff, clients, external providers in both verbal and written form.
• Attending department and agency meetings as required.
• Participate in the on-going assessment of agency health and safety policies.
• Post hold screening, including assessing a client for injuries, providing general first aid, and scheduling appointment with provider for further assessment, if needed.
• Timely and accurate documentation of post hold screenings to ensure continuity of care.
• Intake assessments, nutritional screening, suicide screening, and tuberculosis screening as requested based on need.
• Assist as needed with medication management, including medication entry in BestNotes and e-prescribing, verification and review of medications with pharmacy in prevention of errors, medication re-packaging, medication re-labeling, monitoring prescribed medication along with over the counter medications for cottages, answering questions and or concerns around client medications along with providing assistance with identified medication discrepancies, configuration of medication, checking cycle medications (28 days supply) when medication is received from a pharmacy, distribution of medication when cycle delivery is due, write orders for medication changes when received from assigned licensed psychiatrist, notification to approved client guardian of medication changes, and medication preparation for donation and destruction.

Provide complete, thorough, and detailed guidance with staff, clients, and agency leadership. Attend, lead, and contribute to meetings as needed. Coordinate and communicate with internal and external departments/individuals on client issues. Listens actively and understands others, finds solutions when problems arise and offers alternative solutions, when necessary, Asks appropriate questions when not sure of job requirements or duties.
• Initiative – Complete assigned tasks and duties successfully with little to no supervision in a timely manner.
• Leadership – Practice and model behavior consistent with agency’s code of professional conduct and other policies while also demonstrating ethical practice.
• Professional/Occupational – Demonstrate appropriate behaviors and expectations. Lead by example. Familiar with, interprets, and follows agency policy and state laws regarding employment, workplace, and clients.

Physical Demands and Work Environment:
Moderate physical demands performed both indoors and outdoors, with exposure to changes in weather and fluctuations in temperature. Position requires physical activity and exertion including periods of walking, standing, bending, riding, and moderate to heavy lifting to 50lbs. Minimum Requirements:
Education and Experience and Background:
• Minimum 21 years of age at time of hire.
• High School diploma or GED required.
• Clean federal and state background check.
• Clean WY and Out of State (if applicable) Abuse & Neglect Registry.
• Driving record of no more than two moving violations or at fault accidents in any combination in the past three years, and no major moving violations within the past three years.
• Experience with a variety of MS Office programs and ability to use agency specific technology.
• Proven background in interpersonal, organizational, time-management, oral and written communication, and teamwork required.
Preferred Qualifications:
• LPN, CNA, MA Licensure/Certificate, or degree in a health-related field preferred.
• Two years of experience working in a medical office preferred with administrative experience involving file management, electronic tracking, and substantial documentation volume.
• Experience working with youth and families with mental/behavioral health needs.
