A professional position that is responsible for monitoring, evaluating and collaborating with affiliated providers to ensure quality of Case Coordination activities provided to individuals with intellectual and/or developmental disabilities (IDD) and served within the Sedgwick County Community Developmental Disability Organization’s (SCDDO) catchment area. The TCM Coordinator provides oversight to ensure that Targeted Case Management agencies are delivering quality services while adhering to local policies, State polies, and regulations. They facilitate the SCDDO funding committee, to include review and preparation of all documents associated with each funding source.
Service Provision Oversight
- Review weekly funding requests for completeness and follow up with affiliate TCM staff as needed to ensure compliance with guidelines
- Facilitate weekly funding committee meeting; provide administrative support to Operations staff by completing documentation related to funding requests
- Complete additional funding follow- up per committee decision
- Plan and facilitate local meetings with Directors of TCM services
- Examine and develop solutions with TCM’s and TCM directors to ensure continuity of care
- Provide oversight for individuals who receive local IDD services and do not have licensed TCM services
Continuous Quality Improvement
- Monitor regulatory compliance through the completion of TCM file reviews to evaluate person centered planning, completion of documentation and coordination of services
- Identify trends related to file review outcomes; assists providers in the development and implementation of Agency Intervention Plans; and monitors success.
- Collaborate with affiliate providers during quality assurance meetings to address quality of service questions and concerns
- Collaborate with local and state staff to ensure quality service delivery of all TCM providers
- Collaborate with managed care staff and community stakeholders
- Collaborate with affiliate providers to determine staff development needs of TCM staff; participate with training committee to develop on-going education and support to TCM’s
- Initiate TCM satisfaction survey when necessary, review results and communicate concerns for all individuals with TCM services
- Process complaints or concerns related to TCM services to include appropriate documentation and follow up with providers
Contract Compliance
- Monitor TCM providers compliance with SCDDO Policy Manual
- Monitor compliance with the TCM Rules of Conduct
- Monitor TCM training requirements and communicate additional training opportunities as needed
- Ensure TCM providers adhere to regulatory guidelines when providing services
Minimum Qualifications: Bachelor’s degree in the human services field or similar field of study. Three years of case management related experience, program management experience and/or experience working in a managed care environment. Applicable knowledge of Microsoft Outlook, Word, Excel and Access. Access to reliable, personal transportation. Per Sedgwick County Policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must submit to criminal, abuse/neglect registry background checks.
Preferred Qualifications: Experience working with individuals with intellectual and/or developmental disabilities and supervisory experience.
