Job Description
Leads teams of Care Management professionals providing services Responsible for measuring staff performance. Responsible for managing strategic relationships with client groups and community leaders as needed.

Job Responsibility

  • Teams with a wide range of professionals including psychiatrists, nurse practitioners, community support specialists, nurses, on-site pharmacist, social workers, licensed professional counselors

Job Qualification

  • Master’s Degree in Nursing or related field required, or equivalent combination of education and related experience. Bachelors or Masters must be in Nursing.
  • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
  • 8-12 years of relevant experience and 7+ years of leadership / management experience, required.
