Some of the many programs and services offered are career counseling, skills training, education and literacy programs, employment services, and more.
Position Overview
Provide a wide breath of individualized services to TA recipients to move them toward self-sufficiency. Maintain contact with clients no less than every 30 days, conduct comprehensive initial intake and orientation to available services, make referrals, provide/arrange for supportive services and maintain database. Work as program and system data entry support to Financial Coaches and Life Coaches. Ability to read and appropriately interpret and implement state and local policy is a must. Establish professional relationships; develop/maintain individual plans including continual next-step planning. Complete agency paperwork and data entry in a timely manner. Be willing to make home visits and deliver services in the field.
Essential/Primary Functions
- Send the required letters and correspondence.
- Perform case management duties such as conducting timely intake and orientation on all referrals.
- Perform “real-time” data entry and ensure paper documents are submitted timely.
- Arrange for and process supportive services.
- Collect and process attendance logs.
- Maintain contact with consumers no less than three times a month.
- Establish and maintain professional, confidential, ethical, helpful relationships with all consumers.
- Assess consumers for appropriate levels of service (including waivers) and referrals to internal and external services and providers.
- Place consumers in appropriate activities including community services.
- Develop and maintain good working relationships with internal and external partners.
- Develop and maintain knowledge of community resources.
- Make referrals, advocate, and follow-up.
- Model workplace appropriate attire and behaviors.
- Must travel to surrounding counties within the service area to transport clients and deliver services in the home.
Qualifications
Requirements
- High School or equivalency required.
- BS/BA in Social Work or related field preferred in social services or related field.
- 2-years of experience working with a diverse population in helping, mentoring, coaching, and building relationships.
- Valid driver’s license, current auto insurance, and independent transportation required.
