Licensed Master Social Worker 2 (LMSW 2)

Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) and have one year of post NYS licensure professional social work experience;

OR

Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying.

** Professional social work experience involves activities such as screening, assessing and diagnosing individuals’ and families’ psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying.

Continued employment as a Licensed Master Social Worker 1 or 2 is dependent upon current licensure and registration by New York State as a Licensed Master Social Worker or Licensed Clinical Social Worker. Loss of licensure or registration may result in removal from employment.

Duties Description POSITION DESCRIPTION:

The Office for People With Developmental Disabilities (OPWDD) provides high-quality, person-centered services, supports and advocacy to approximately 136,000 individuals with developmental disabilities in New York State. The Office for People With Developmental Disabilities (OPWDD) Division of Service Delivery Regional Offices seeks a Licensed Master Social Worker 2 who will act as the Residential School Transition Coordinator (RSTC) within the Age Out Unit of DDRO Region 4. The Age Out Unit is responsible for transitioning OPWDD eligible individuals in residential schools and in foster care to adult residential and day services. The Residential School Transition Coordinator (RSTC) plays a critical role within the Age Out Unit’s planning process.

PRIMARY DUTIES AND RESPONSIBILITIES:

The candidate selected for this position will act as the Residential School Transition Coordinator (RSTC). The incumbent’s duties will include but are not limited to the following:
• Ensure that individuals aging out of the residential schools receive appropriate OPWDD adult services.
• Assist with state management and reporting for individuals transitioning from residential schools to OPWDD adult services using Excel, Citrix, and relevant databases.
• Participate in weekly and monthly meetings, both in person, by phone or by video conference with agencies, families, residential schools and developmental staff, all meetings relate to the transition of transition individuals from residential settings to OPWDD adult services.
• Work closely with the Eligibility Team to ensure those in residential schools meet OPWDD criteria to receive services.
• Follow the Front Door process including updating the Eligibility Assessment and Authorization system.
• Provide information to providers on the Higher Needs Funding process, including administering or updating the DDP2.
• Visit residential Schools both in state and out of state for upcoming graduates as needed.
• Maintain dialog with families, both in writing and over the phone regarding their child and the placement process.
• Review Preliminary Adult Services Plan/Behavioral Plan to ensure the needs of the individual will be met by the OPWDD Adult Residential Opportunity. If the plan is not approved, work with the agency to make corrections, deletions, or additions.
• Send referrals for residential placement and Day Services to appropriate agencies.
• Will cover assigned cases within NYC Region 4.
