Johnson County’s Department of Human Services has a full-time open position for Accessibility Case Manager. First review of applications for this job will begin on July 10, 2023. The accessibility case manager is responsible for conducting intake assessments with those who are age 59 years or younger and homebound to determine eligibility for accessibility services (homemaker and meals on wheels). The intake assessment takes place in the individual’s home or via a phone interview. The accessibility case manager maintains a case load of 20-25 individuals for case management follow up, and recertification assessment every 6 months. This position also works within the Multi-Service Centers for additional support. The majority of the intake assessment and follow up case management is completed with use of mobile technology during the home visit.
- Conducts assessments of homebound individuals’ background and applicable circumstances to identify needs and/or determine service eligibility; interprets and communicates relevant information to individuals, families, and other stakeholders.
- Develops plans to address the needs of individuals in single or multiple areas of focus; networks with other community agencies to research and coordinate a variety of resources and services to address short term intensive or long term individual needs; encourages the establishment and maintenance of support systems; consults and collaborates with other providers in the development, implementation and review of plans.
- Provides a variety of services for a designated case load of homebound individuals with multi-faceted and/or ongoing needs; ensures services provided are consistent with individual plans; and occasionally provides services in a community based setting.
- Completes and submits documentation regarding services provided; evaluates and documents individuals’ progress according to measurable goals; reviews quality and effectiveness of services provided.
Job Requirements
Bachelor’s degree in Human Services or relevant field and one year of related work experience with the focus on aging and/or individuals with a disability; including assessing, intervening, and evaluating clients in complex situations; and a valid driver’s license with an acceptable driving record are required.
A Master’s Degree in Human Services or three years of relevant work experience is preferred.
