As an Administrative Support Clerk on the Health and Information Management (HIM) team, you will assist with collecting data, analyzing clinical records, and handling releases of confidential information. This position also helps with admission and discharge recording, collection of statistical data, and preparing related reports.

Job Description Summary:
- Assist with collection and compilation of data for statistical reports by manual or automated systems for use by administration, medical staff, other departments, and governmental agencies
- Assist in maintaining systematic flow of clinical information to and from all departments of the facility. Assist with retrieval of information and preparation of reports for administrative and professional staff
- Respond to request for clinical information in accordance with facility policy and procedures
- Follow policies and procedures designed to assure confidentiality of health information
- Audit medical records for qualitative and quantitative completeness based on facility policy and accreditation standards
- Maintain records for discharged Veterans in an organized fashion according to professional standards. Prepare discharged records for long term storage. Assist with destruction of discharged records according to government regulations and facility policy
- Assist with preparation of records for presentation in court in response to subpoena duces tecum, serves as a witness as required
- Initiate medical records for new admissions/re-admissions, as assigned
- Assist with oversight and or coordination of visits for health care professionals ensuring appropriate documentation is entered into the Electronic Health Record (EHR). May assist with assigning and managing EHR access levels for employees, health care professionals and surveyors
- Assist with maintenance/data entry of electronic health record, to include but not limited to: scanning of paper records, initiation of medical record for new admissions/readmissions, icon identifier management, provide census management oversight, etc.
- Will assist in maintaining required hard copies of Veteran’s information as directed
- May assist with scheduling Veterans clinical appointments
- Maintain Veteran confidentiality
- Provide reports, lists and worksheets as needed
- Lift and/or move materials, as necessary, in the performance of assigned duties
- Demonstrate proficiency in the use of computer and programs utilized by the facility. Perform manual electronic data entry
- Answer phone in a courteous and professional manner relaying accurate messages
- Adhere to infection control procedures as trained
- Participate in the care planning process as directed
- Educate staff on confidentiality issues and use of medical record software. Assist with orientation and educational programs for facility personnel in those phases of medical records applicable to their field of work
- Schedule Veterans Care appointments as needed and coordinate with Driver
- Other duties as assigned
