Job Description:
The primary purpose of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. Assist in the review of potential resident admissions information.
Essential Duties (include but are not limited to):
- Assist the Social Service Director/Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.
- Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
- Assist in interviewing residents/guardians/sponsors and obtaining required admission information and signatures.
- Admit, transfer, and discharge residents in accordance with established policies and procedures.
Qualifications:
- Must be able to read, write, speak, and understand the English language.
- Must have a High School diploma or equivalent.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
- Must be a supportive team member, contribute to and be an example of team work and team concept.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
- Must possess leadership ability and willingness to work harmoniously with other personnel.
- Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents
