Behavioral Health Case Manager II
Schedule: Mon – Fri | 8:00a – 5:00p HST
Remote (30%) + Fieldwork (~70%) – must reside on Island of Hawaii – Kona Side. Fieldwork coverage includes Island of Kauai and Hawaii Island. They are never required to go into the office. Only work from home and fieldwork in the community with members.
Sign-on Bonus: $5,000 – half up front, half after 6 months
Position Summary:
Behavioral Health Case Manager is responsible for coordinating and managing the care of individuals with behavioral health conditions. You must have a combination of clinical, interpersonal, and organizational skills to provide effective care to individuals with behavioral health conditions.
Minimum Requirements:
- Requires Master’s Degree in social work, counseling, a related behavioral health field, or a degree in nursing.
- Minimum of 2 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
- Current active unrestricted license such as LMFT, LCSW, LSW, RN, or LMHC to practice as a health professional within the scope of licensure in Hawaii is required.
- Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders.
- Managed care experience required.
Preferred Skills, Capabilities, and Experiences:
- Experience in health coaching and motivational interviewing techniques preferred.
- Assessment and evaluation skill.
- Communication and interpersonal skills.
