Behavioral Home Health Specialist

POSITION SUMMARY: Provide BHH services which focus on developing individualized goals with consumers around a valued role that will increase their independence, success and overall quality of life. This will be accomplished through intensive skill building and motivational support for pursuing goals related to health and wellness, employment, education, community involvement and use of natural supports.

Accountable and responsible for the following results:

  • Collaborates with Nurse Care Manager and other team members to provide individualized services and support.
  • Assist individuals in setting goals and following through on wellness and health activities.
  • Assist individuals in meeting wellness and recovery goals and managing chronic illnesses.
  • Assists in the scheduling and coordination of medical appointments.
  • Observe, record and evaluate consumer’s progress towards goals.
  • Document stage-wise interventions according to the psychiatric rehabilitation skill building model.
  • Provide outreach & crisis intervention to community support service consumers as needed.
  • Provide individual and group health and wellness sessions on-site or in the community focused on the appropriate stage-wise intervention.
  • Provide limited transportation as needed while consumers learn individual transportation skills.
  • Provide education, support and consultation to family members, significant others and community members to meet the rehabilitation goal(s) set by the consumer.
  • Develop individualized and generalized community supports.
  • Consult with Integrated Care Supervisor whenever a consumer no longer requires the current level of care.
  • Required to use own vehicle on company business, or to transport clients.
  • Attend training sessions, workshops, seminars as requested by supervisor, or as needed forprofessional development, or as required for accreditation or licensing.
  • Provide direct service in accordance with contractual guidelines.
  • Adhere to the standards of care to ensure that the identified quality measures are obtained.
  • Supply data in a timely fashion to the leadership to fulfill reporting requirements.
  • Perform other duties as assigned.
  • Regular and consistent attendance is an essential function of this position.

Qualifications

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree preferred. In lieu of bachelor’s degree: Associate degree or college coursework equivalent (2 years) and 2 years of experience working with clients with mental health and substance use disorders; OR high school diploma and minimum of 4 years of experience working with clients with mental health and substance use disorders.
  • 1- 2 years’ experience in a community mental health environment preferred. Knowledge of Psychiatric Rehabilitation is desirable.
  • Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
