Who We Need

Immediately Hiring a Housing Program Coordinator for our BHS department in Mesa, AZ. Monday-Friday 8am-5pm.

The Program Coordinator provides quality services to enhance individuals with disabilities live an everyday lifestyle. Services are provided in home settings; apartment living, day programs work related programs and in the community. The Program Coordinator promotes active treatment and a positive environment for individuals as identified in their individualized service plan/behavior plan.

MINIMUM QUALIFICATIONS:

Requires any combination of training, education, and experience listed below:

  • Requires a Bachelor’s degree in Human Services or related field plus two (2) years’ experience working with adults in a residential behavioral health setting: equivalency of education and experience acceptable.
  • One (1) year of supervisory experience in behavioral health setting required.
  • Must have a Level 1 Fingerprint Clearance Card through the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.
  • Required to maintain a valid Arizona driver’s license and are able to pass a motor vehicle screen for insurance purposes in order to transport clients.

JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)

  • Develop and Direct program functions and objectives, collect performance data and performance measurements , implement policies and procedures and recommend program/plan changes;
  • Participate in the hiring, training, coaching, mentoring, and supervising of team members, to ensure all members needs are coordinated and addressed;
  • Ability to complete required staffing reports (timesheets) timely and accurately;
  • Ability to work proficiently with staff schedules, hiring requirements, and HR Training calendars;
  • Must be flexible with providing shift coverage in emergent situations;
  • Timely completion of reports to include: employee evaluations, satisfaction surveys, evacuation drills, facility reviews, and overlap reports;
  • As directed, complete overnight visits to sites;
  • Available by phone 24/7.
  • Complete daily case management documentation;
  • Maintain billable hourly requirements as assigned;
  • Responsible for complete and accurate Assessments, ISP’s, Intakes, and discharges;
  • Complete Monthly Summaries timely.
  • Review client charts (scorecards) ensuring all documents are current and accurate;
  • Complete Incident Reporting and follow up as required;
  • Schedule and facilitate regular staff meetings and Staff Development sessions;
  • Specialty sites: responsible for required data tracking and reporting;
  • Responsible for coordination of care daily for individuals served with Copa staff, clinical teams, guardians , and advocates;
  • As directed, cover shifts in emergent situations;
  • Residential Facilities concurrent reviews completed monthly with Mercy Care Housing;
