Core Responsibilities:
- Develop and Cultivate Internal & External Referral Partners.
- Determine Client Eligibility and Best Fit For Long Term Case Management Programs.
- Complete Enrollment Process With Clients.
- Ensure Data Collection is Completed Properly.
- Produce Weekly Analytics and Data Reports to Reflect Recruitment/Enrollment Trends.
What will you be doing in your role?
- Evaluate the target client population to identify appropriate outreach strategies.
- You identify referral partners and develop strong relationships to reach target client population.
- You proactively manage recruitment partnerships with attention to detail.
- You conduct outreach activities, including presentations, to service providers and potential clients.
- You follow up with individuals and/or families who have expressed interest in services to expedite enrollment.
- You help client families determine best case management fit & complete enrollment process.
- You offer support in reporting timelines and requirements for all internal and external stakeholders.
- You are knowledgeable of eligibility criteria for case management programs
- You maintain and update recruitment and enrollment databases to identify trends in recruitment & enrollment.
- You will also work with clients to guide them through the enrollment process and help them best determine with long-term case management program best fits their needs.
- You will work with program staff to keep informed on program developments and challenges.
Are you the newest Outreach Specialist?
- Bachelor’s degree in social work or related field.
- Twoyears’ Case Management experience, preferred.
- Must be willing to travel to partner meetings as needed.
- Bilingual in Spanish.
