Position Summary: LifeMoves is seeking a highly motivated Bilingual (Spanish Speaking) Case Manager to join our team. Under the supervision of the Program Director, the Bilingual Case Manager will work collaboratively with five community organizations based in Redwood City to support people experiencing unsheltered homelessness living in encampments, vehicles, and the streets. The Case Manager will utilize a multidisciplinary approach to provide intensive case management services to clients with the highest barriers and support their transition toward stable housing. Responsibilities include developing and implementing individual case plans for a caseload of clients, collaborating with community agencies, and assisting the Program Director to develop program services.

Case Management Duties:

  • Provides in-the-field intensive case management services to unsheltered homeless individuals and households who reside in encampments, streets, and vehicles
  • Utilizes Collective Impact framework to meet client needs by communicating effectively across agencies
  • Identifies target population, performs screening assessments, makes appropriate referrals, and provides linkage to primary care, mental health, short term residential stabilization, and other social services
  • Develops, implements and revises individual case plans and consistently monitors progress toward client goals
  • Provides on-going supportive and/or case management functions in accordance with the case/treatment plan
  • Participates collaboratively as a team member in supporting clients and other case managers/outreach workers as needed
  • Maintains client and program records in accordance with applicable standards and regulations, grant requirements, etc.
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
  • Participates in continuing education activities, remaining knowledgeable in area(s) of expertise
  • Participates in program specific community meetings, All-Staff Meetings, program open houses, and other agency-wide functions
  • Other duties as assigned by supervisor

Position Qualifications

  • Bachelor’s degree in social work or related field. Direct services experience may be substituted for academic credentials
  • Prior experience working with individuals who are homeless, at risk of homelessness, who suffer from mental health, and/or chronic and persistent substance abuse disorders
  • Excellent communication and interpersonal skills; proven ability to collaborate with community partners and resolve conflicts
  • Understanding of the issues faced by homeless individuals and other marginalized populations in crisis
  • Availability to work occasional evenings and weekends
  • An understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area
  • Bilingual Spanish language capabilities required
  • Lived experience of homelessness or housing insecurity a plus
