Community Health Nurse II – HEALTH

Pay: $48,214.40 annually | This position may be eligible for a sign on bonus of $2,500 after 60 days of employment if selected applicant meets all policy conditions.

Work Schedule: 40 hours per week. May exceed 40.

Provide direct in-home prenatal and parenting education and support to women and families in Sedgwick County with a focus on the reduction of infant mortality, low birth weight, and prematurity among high-risk mothers. Education and support is both provided at in home and doctors office visits as provider will accompany clients to Well child visits assisting with naviation in the healthcare system.This position works to meet the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program’s focus on reduction of infant mortality, low birth weight, and prematurity among high risk moms.

Home visits

  • Complete in-home visits following program guidelines.
  • Evaluate clients for mental and physical health barriers by performing a variety of assessments.
  • Ensure a Welcome Home visit is completed once client has delivered.

Documentation

  • Documentation will occur in real time at client visits, with full completion in 48 hours.
  • Audit documentation, revise, and return to coordinator by deadline given.
  • Complete documentation according to policies/procedures.

Education

  • Provide education on maternal child health, smoking cessation, Substance use cessation, child safety, family planning, mental health, and other resources, as applicable.
  • Conduct developmental sceenings per curriculum timeframe.
  • Complete referrals to other community agencies based on individual client needs.
  • Participate and represent Healthy Babies in a professional manner by conducting presentations at health fairs, community events, coalitions, and for other community agencies. Required to participate in 3 community events in addition to the Community Baby Showers hosted by Healthy Babies per year.

Minimum Qualifications: Associate’s Degree in Nursing. One year experience in public health, labor and delivery, postpartum or pediatric nursing. Current Registered Nurse (RN) license in the state of Kansas required. Attends required trainings to maintain licensure and/or certification. Per Sedgwick County policy, this is a driving level position that requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance.

Preferred Qualifications: Bachelor’s Degree in Nursing (BSN). One year experience post BSN. Home visiting experience. A minimum of one year working in a hospital setting. Current Registered Nurse (RN) license in the state of Kansas required. Attends required trainings to maintain licensure and/or certification. Bilingual English/Spanish skills. Knowledge in early childhood development and prenatal education. Current CPR certification.
