Description

POSITION: Care Coordinator SITE: Residential Services

REPORTS TO: Care Coordinator

GENERAL FUNCTION:Provide coordination of health care services and daily living needs for youth in Casa de Amparos residential treatment center. Collaborate with Consulting Psychiatrist and community-based physicians and clinics to coordinate care and individualized treatment for residents. Coordinate and attend doctor appointments with clients, transporting in agency vehicles as needed. 8-hour shift, 5 days between the hours of 6:30 am-7:30 pm, may include weekends. This may require staff to provide supervision and transportation of youth and ensure their safety during appointments.

QUALIFICATIONS:

  • Must be able to be associated with Casa de Amparo through Community Care Licensing (CCL), which typically includes clearance by FBI, DOJ and CACI.
  • Education and/or experience requirements:
    • Have a Bachelors Degree (in human science field preferred) OR
    • Have a valid Child Development Teaching Permit OR
    • Have completed 12 semester units of Early Childhood Education, Adolescent Development or Foster and Kinship Care Education and have at least 100 hours of experience working with youth OR
    • Have a valid certificate as an Alcohol Counselor, Drug Counselor or Alcohol and Drug Counselor and have at least 100 hours of experience working with youth OR
    • Have a valid vocational training certificate, credential or documentation demonstrating that the individual is a trade journeyperson who instructs children in vocational skills and have at least 100 hours of experience working with youth as a mentor, athletic coach, teacher, vocational coach, tutor, counselor or other relevant experience as determined by the department OR
    • Have previously been employed as a full time staff or served as a volunteer at a group home, STRTP or substance abuse treatment program for at least one year OR
    • Have relevant life experience in the child welfare, mental health or juvenile justice systems as a consumer, mentor or caregiver or other relevant experience as determined by the department.
  • Knowledge of developmental needs of children ages 0-18 years throughout the developmental stages preferred.
  • Must possess maturity, good judgment and the ability to be a positive role model for children.
  • Two years experience in a behavioral health care or nursing setting preferred.
  • Knowledge of psychotropic and general medications and effects on children preferred.
  • Knowledge and experience working with MediCal, Community Care Licensing (CCL), The Joint Commission, and the Health and Human Service Agency system of care preferred.
  • Experience providing crisis intervention with assaultive or aggressive teens.
  • Must have the physical ability to participate in activities and perform related physical skills, including, but not limited to, crouching, lifting 50 pounds, crawling on hands and knees, walking up and down stairs, and having the ability to effectively, physically intervene with teen residents whose psychological instability can lead to unexpected aggression and/or unsafe behavior with little provocation resulting in erratic behavior.
  • Ability to comply with all employment requirements including, but not limited to CPR/Pediatric 1st Aid, physical, TB and drug screen clearance, clear DMV record and approval from insurance carrier to drive agency vehicles. Must provide proof of immunization against influenza, pertussis, and measles.
  • Must be at least 22 years of age.

MAJOR FUNCTIONS:

  1. Under the supervision of the Health Center Manager, coordinate medical, dental, ophthalmologic and other medical care appointments for residents.
  2. Ensure appropriate documentation is provided to Public Health Nurses per contract requirements.
  3. Collaborate with Lead Care Coordinator, Psychiatric Nurse, Consulting Psychiatrist, Clinical Director and community-based physicians and Public Health Nurses.
  4. Ensure accurate Medical Appointment materials are maintained for all youth.
  5. As needed, support Residential Services staff in the cottages including, care and supervision and with the assisted self-administration of residents medication. Within this scope of responsibility, the Care Coordinator would need to meet the requirements of the Qualification segment of this job description detailing the need to be capable of physically intervening with escalating residents.
  6. Provide input for resident treatment planning through ongoing communication with health care and clinical staff and appointment documentation.
  7. Participate in Residential Services staff meetings and Treatment Team meetings.
  8. Assist in collecting information for the Medical Transition Summary, Discharge Summary, Quarterly Youth Status Report, and ensure updates are made to the Health and Education Passport prior to a residents transition.
  9. Maintain requisite paperwork in a timely and professional manner according to Community Care Licensing, county contracts, accreditation standards and/or other contract requirements.
  10. Assist in maintaining medication cart inventory in all cottages, as appropriate.
  11. Assist in maintaining the daily operations and cleanliness of the Health Center.
  12. Provide sole supervision of youth during medical appointments and transports.
  13. Performs special projects or assignments as assigned by Supervisor.

EFFECT ON END RESULT: This position assures that required health care services for youth are appropriately documented and coordinated in a trauma informed, non-judgmental, professional and timely manner.
