Description

Position Summary:

Person in this position will provide veterans and their families with linkages to community resources and case management to ensure their access to veterans benefits, medical assistance, housing services and other needs they may experience. This will include arrangement, coordination, monitoring, and delivery of services related to meeting the needs of the clients and helping them access community services and obtain employment, housing stability, medical care and more. Essential Job Functions include those listed below. Other duties may be assigned as needed.

Essential Job Functions:

  • Oversight of day-to day operations of the program
  • Builds and maintains relationships with local veterans providers, the VA Medical Center, advocacy groups property owners/managers to secure units for leasing as needed
  • Meet with each client at least once weekly to provide support and assistance towards achieving each Individualized Service Plan
  • Intake and assessment of incoming participants to determine appropriate services.
  • Make referrals and appointments with appropriate agency, services and/or case managers.
  • Maintain client files according to company policy and contract requirements.
  • Provide daily, weekly, and monthly reports as requested.
  • Assist and monitor data collection and statistical reporting and complete Quality Improvement reports as requested.
  • Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested.
  • Coordination of various services throughout several agencies.
  • Attend scheduled meetings and presentations with referral agencies; and
  • Perform any other duties as assigned.

Qualifications

Essential Qualifications:

  • Excellent communication and public speaking skills.
  • One (1) year supervision experience, preferred.
  • Two (2) years case management or counseling services.
  • Demonstrated ability to work with the underserved populations.
  • Ability to conduct outreach, including street outreach
  • Good computer skills.
  • Good documentation skills.
  • Valid drivers license and ability to obtain and maintain WestCares vehicle insurance is required.
  • Be able to multi-task and make independent decisions; and
  • CPR Certification, First Aid Certification, and an Annual Tuberculosis test.
  • As mandated by Presidential Order all employees of WestCare Foundation, Inc. and all affiliates of WestCare Foundation, Inc. will be required to be fully vaccinated against COVID-19 effective 12/08/2021.

Education:

  • Graduation with a two-year degree in Psychology, Social Work, Community and Human Services or a related field is preferred; and
  • High School diploma or equivalent is required.
  • 2 years experience in working with veterans
  • Knowledge of veteran services and local resources

Experience and Competencies:

  • Valid drivers license and ability to obtain and maintain WestCares vehicle insurance is required.
  • Must be able to obtain and maintain Fresno County Credentialing; and
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test. As mandated by Presidential Order all employees of WestCare Foundation, Inc. and all affiliates of WestCare Foundation, Inc. will be required to be fully vaccinated against COVID-19 effective 12/08/2021. Employees that have requested a Medical or Religious Exemption, which has been approved by Human Resources will be required to test for the virus weekly.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
