CASE MANAGEMENT SERVICES DIRECTOR

Customized Assistance Services (CAS) helps Human Resources Administration (BRA) clients with health and/or mental health problems reach the highest attainable level of functioning and self-sufficiency by providing comprehensive, integrated, individualized clinical and support services. CAS works with other components of HRA and with other governmental and non-governmental service providers to create new programs and to integrate and refine existing services so the people it serves can achieve their maximum functional capacity. CAS provides clinical expertise, recommendations, and direction to HRA in the fields of health, mental health, substance abuse and vocational rehabilitation.

CAS staff provides psychiatric evaluation and crisis intervention services, housing eligibility services and assistance in obtaining federal disability benefits. CAS manages contracts that serve individuals and families with medical, mental health, and/or substance abuse disorders. CAS programs provide clinically focused case management and utilize sophisticated clinical tracking and reporting systems. The Case Management Services Director will administer, coordinate, and monitor SUPPM Case Management (CM) Programs which provide for the delivery of comprehensive CM services for all populations designated by HRA.

The Customized Assistance Services is recruiting for (1) one Case Management Services Director who will:
• Administer, coordinate and monitor SUPPM Case Management (CM) Programs which provide for the delivery of comprehensive CM services for adult populations designated by HRA (substance abuse disorder; recurring homeless or risk of homelessness; diagnosed or suspected severe medical, mental or behavioral health conditions). CM services will include the completion of psychosocial assessment and service plans, case coordination with HRA offices, and treatment and any other service providers who assist clients in maximizing health, income, and housing stabilization.
• Collaborate with HRA’s Homelessness Prevention Administration (HPA), Investigation Revenue & Enforcement Administration (IREA) and the NYC Mayor’s Office of Operations staff to plan and implement activities/services necessary to relocate residents of three-quarter housing units to approved transitional housing locations. Review CM documentation and make recommendations to senior management for transitional housing resident transfers or discharges and compile all pertinent information and documentation to support recommendations.
• Review specific troublesome case issues with CM vendors, HPA and other HRA offices as they arise with the intention of maintaining housing and any other services for the subject population. Offer creativity and innovation in the design and development of policies and programs that increase the self-sufficiency, employability, and wellness of clients with disabilities.
• Ensure that the Substance Abuse Centralized Assessment Program (SACAP) completes substance-use assessments timely with potential volunteers for relocation thus ensures that the subject population is re-housed and not put at risk for homelessness.
• Work closely with HPA and other parts of the agency to ensure effective communication and coordination of care between HRA’s case management vendors and those vendors which provide rapid re-housing services to transitional housing residents.
• Develop and implement standard operating procedures and reporting mechanisms for monitoring the effectiveness and timeliness of Case Management service contractors/providers.
• Facilitate operational meetings involving CM service providers and relevant HRA departments to enhance the exchange of information and to ensure effective service delivery.
• Produce reports to help evaluate the program’s effectiveness in meeting its goals while ensuring a high quality of service to clients.

Minimum Qual Requirements

1. A baccalaureate degree from an accredited college or university and four years of progressively responsible experience, including one year at the administrative or managerial level in a large governmental agency, business firm, civic or community organization operating in the area of social services; or

2. Education and/or experience equivalent to “1” above. However, all candidates must have a baccalaureate degree from an accredited college and the one year of experience at the administrative or management level as described in “1” above.

Preferred Skills
• Strong administrative and management skills.
• Subject matter expertise in at least one and preferably a number of the following areas: health, mental health, substance use disorders, vocational rehabilitation, case management.
• Experience working with underserved populations such as substance users, veterans, homeless individuals, ex-offenders and LGBTQI.
• Experience designing and implementing policies and programs that increase self-sufficiency, employability, and wellness of clients with disabilities,
• Experience working with contracted human services programs
• Ability to work across agency and City organizational lines within multidisciplinary initiatives
• Excellent writing and presentation skills
