Description

JOB SUMMARY:

The Case Manager/QIDP reports to the Associate Director of Case Management in accordance with stated agency policies and procedures. The employee will focus on three key pillars: to advocate, communicate, and coordinate with and for the individuals served at Helping Hand.

KEY RESPONSIBILITIES:

  • Promote the individual’s personal development and quality of life in a manner that is determined by the individual and his/her support team in an environment conducive to the qualities of freedom, informed personal choice, and participation. Promoting the least restrictive environments, optimal community integration, and personal safety are of paramount importance.
  • Provide individualized and person-centered services to the individuals served and ensure that their rights are promoted and protected across all environments. Be an advocate for the individuals served.
  • Act as the liaison to coordinate services for individuals served across internal and external departments which include, but are not limited to Families, ISC and Provider Agencies, DHS, Medicaid/Medicare, Medical Providers, Schools, Hospitals, Rehabilitation Facilities, Therapists, etc.
  • Ensure that documentation as part of the Implementation Strategy processes is completed on time in accordance with DHS regulations and Helping Hand’s policies and procedures.
  • Based upon the services being delivered to the individual, assist the individual in accessing and maintaining funding, transportation services, employment, medical services, community integration, and home-based waiver services.
  • Regularly maintaining and updating the files of individuals served in accordance with Helping Hand’s “File Review” standards as well as DHS standards.

Qualifications

STANDARD JOB REQUIREMENTS:

  • Display ethical and professional conduct.
  • Lay a foundation in support of personal growth based on Helping Hand’s standard of the principle of performance that everyone can grow in one or more personal aspects such as physical, mental, and/or emotional, and that he/she has abilities that can be maximized.
  • Be knowledgeable of the individual’s life, as they relate to services and supports that have been contracted with Helping Hand.
  • Provide safety and security of individuals along with Helping Hand property.
  • Adhere to HIPAA, confidentiality, and Helping Hand policies and procedures.
  • Maintain a regular work schedule of M-F, along with being available and dependable during varying hours including evenings and weekends in accordance with the position’s need.
  • Understand and implement laws, professional standards, and Social Security and Public Aid regulations regarding people with disabilities.
  • Communicate all individual needs appropriately and conduct home visits when applicable.
  • Train and support through individualized plans.
  • Ability to promote and adhere to company values.
  • A commitment to a diverse, inclusive, and equitable work environment.

QUALIFICATIONS:

  • Bachelor’s degree in social work or a related field.
  • At least one year of experience working with individuals with intellectual disabilities.
  • QIDP Certification (preferred).
  • Exceptional interpersonal and communication skills, both verbal and written.
  • Exceptional problem-solving and time-management skills. Ability to work in a fast-paced environment with the flexibility to adapt to changes throughout the day.
  • Basic computer skills, proficient in Microsoft Word and Microsoft Excel.
  • Valid driver’s license with an acceptable driving record and ongoing motor vehicle insurance. Ability to pass PACE certification requirements (preferred).
