*This is a part-time position at 20 hours per work week and assigned at the Alhambra Parks and Recreation Department*
Under general supervision, the Case Worker assists with all aspects and components of the Alhambra Case Management Project; evaluates the potential client and identifies the functional and other limitations that impede independent living; delivers all care management services; and performs other related duties as assigned.
SUPERVISION RECEIVED AND EXERCISED:
Supervision is provided by the Senior Services Supervisor.
Essential Job Functions
Essential job functions may include, but are not limited to, the following: (These functions may not be present in all positions in this classification. Performs and conducts intake screening and comprehensive assessment of elderly clients’ psychosocial and health needs; prepares individualized care planning to include the client and significant others whenever possible; services authorization/arrangement through coordination with local agencies; collects and organizes accurate and proper documentation on individual case client; maintains confidentiality of client, statistical reports, and record keeping as required by the City and Los Angeles County Area Agency on Aging; develops strong professional working relationships with existing state, county, local agencies, health care professionals, and community organizations who provide services for the elderly; participates directly in periodic case reviews of representative and difficult clients; attends regular Project staff and Senior Services meetings as well as those in the community; participates in the planning and development and distribution of marketing materials; attends Case Management Advisory Board meetings working with members on fundraising and community awareness projects; coordinates provision of services with other departments/divisions; and utilizes an accurate and updated Information and Referral Directory as well as other resource information to refer inappropriate clients to the other agencies and services.
Qualification Guidelines
To qualify for this classification, an individual must possess any combination of experience and education that would likely produce the required knowledge and abilities, and enable the individual to successfully perform the essential job functions of the position. A typical combination is:
Education and/or Experience:
Bachelor’s Degree in social services, human services, or gerontology, with two years of responsible experience in care/case management with older adults in a social service agency setting and experience relating to elderly clientele on a one on one basis.
Knowledge of:
Human behavior and the aging process; community resources and available funding sources; quality of services recommended; social and health intervention techniques; mental and physical problems affecting the frail elderly; and the capabilities and limitations of older adults.
Ability to:
Treat clients, family members, service providers, and co-workers with respect and dignity; communicate effectively with clients, family members, service providers and co-workers, both orally and in writing; keep detailed and confidential client records; and deal effectively with all levels of staff, other City Departments, individuals in local community agencies and organizations, health care professionals, and the public.
Bilingual (speaking, reading, and writing) in Mandarin Chinese, Cantonese Chinese and/or Vietnamese is highly desirable.
Licenses and Certificates
Possession of and ability to maintain a valid California Class “C” Driver’s License is required.
SELECTION PROCESS:
Applicants must complete an application online prior to the final filing date. Successful candidates must achieve a minimum passing score in each of the test components. Applicants will be notified by e-mail as to the test date, time and location. The examination process will include:
Oral interview weighted at 100%
Candidates will be required to pass a City administered pre-employment physical and may be required to pass a thorough background investigation and polygraph examination as a part of the selection process. Candidates who do not pass will be disqualified and removed from the eligibility list.

Effective January 1, 2013, new members to CalPERS or an agency with CalPERS’ reciprocity will be subject to the provisions of the Public Employees’ Pension Reform Act of 2013 (PEPRA) and will receive 2% @ 62 benefit formula underwhich the employee pays 7.75% as a contribution to the PEPRA plan. Employees who are current members of CalPERS or an agency with CalPERS’ reciprocity, or who have less than a six-month break in service between employment in a CalPERS (or reciprocal) agency will be enrolled in the 2.7% @ 55 benefit formula underwhich the employee pays 8% of the employee contribution to PERS. The City offers a Section 125 cafeteria benefit plan. The City does not participate in the Social Security System. City paid leave time (sick, vacation and holiday) available in accordance with City policies.
