What will you be doing in your role?
- You will work collaboratively as part of a case management team to ensure completion of client assessments and development of a service plan that is realistic and achievable.
- You will provide Financial Assistance (FA) and connection to resources by assessing client needs and goals as well as coordination of services with internal and external resource providers.
- You help clients connect with valuable resources by advocating with community partners and by providing emergency financial assistance as needed.
- You will ensure professional delivery of service through excellent rapport and communication with client and community partners. Completes all necessary documentation and reports, as well as participate in regular client staffing’s with internal and external partners.
- You are informed about community resources; maintain current knowledge of practices and issues around poverty and family self-sufficiency and share with your team.
- You monitor program due dates to ensure all required targets are met.
- Completes all necessary documentation and reports, as well as participate in regular client staffing’s with internal and external partners.
- You help your team apply a problem-solving framework that promotes steady client progress.
- You support the data and evaluative needs of the program by ensuring proper documentation and data input is completed.
- You incorporate use of all available resources, drawing on CCFW’s internal and external partnerships in order to help clients progress with a holistic service plan.
- You will float from our Arlington, Fort Worth, and Colleyville campuses as needed by the program.
Are you the next Client Navigator?
- Bachelor’s degree in social work preferred or combination of experience and education
- Work experience in case management is preferred
- Experience with other community agencies is preferred
