ABOUT THE ROLE
Yuvo is hiring a Clinical Quality Outreach Manager to join our Population Health Team.
Yuvo Health is seeking a Clinical Quality Outreach Manager who has experience working with various internal and external stakeholders to build and manage an outreach program focused on improving quality metrics through digital campaigns (eg. Well Child Visits, Breast Cancer Screening). This position will be responsible for assessing current state efforts with new health care centers and payers and developing and facilitating improvement efforts to fill care gaps to achieve quality targets. Individuals in this role will need to work closely with internal colleagues to ensure a collaborative approach to driving improvement towards quality targets. The successful candidate will have strong skills in patient outreach systems, quality improvement, project management, and is highly organized. Experience working in a health care setting with a focus on quality improvement desired. The Clinical Quality Outreach Manager reports to the Director of Quality.
This is a full time, 100% remote position for employees based in the United States with minimum required availability from 10:00 am EST to 3:00 pm EST.
Travel up to 25% to work directly with your designated FQHC provider partners if needed.

WHAT YOU’LL DO
- Manage internal and external partner relationships across multiple departments and external organizations.
- Management and oversight of automated population health solutions and electronic systems reporting.
- Research and incorporate market dependencies which will impact potential campaigns
- Develop a portfolio of digital campaigns to be used in a standardized way with the ability to incorporate new partner dependencies.
- Effectively organize multiple active campaigns with the ability to monitor continuous progress
- Utilize quality data to target priorities for campaigns, and thoroughly track and report on progress toward operational improvement goals.
- Develop and implement operational workflows to ensure successful implementation, scaling and sustainability.
- Utilize standard project management tools and principles to define and manage project scope, monitor timelines and deliverables, and communicate and identify pathways to resolve risks and barriers.
- Proactively and creatively address any challenges that may arise during project execution.
- Partner closely with health centers as well as performance managers to drive positive outcomes
- Hire, train and supervisor employees which support the clinical quality department goals.
- Any other duties are reasonably required.
Requirements
WHAT YOU’LL BRING (REQUIRED)
- Bachelor degree in a health related field
- Quality Improvement and project management training/experience
- Experience working in or with primary care organization and conducting outreach to improve engagement in care
- Experience facilitating teams in improvement initiatives
- Experience working with various types of health organizations
- Highly organized with ability to document progress in an absorbable way
- Familiarity with Value Based Care model

Base Salary Range: $100,000 – $120,000/year
