Why does the job exist?

The Community Alignment Specialist (CAS) will manage, coordinate and participate in the development of community resources in partnership with both public and private agencies within the community, and oversee the development and implementation of a marketing plan for the community. This position is responsible for the establishment of viable coalitions and partnerships among civic, community and health and human services partners to plan programs, services and supports designed to meet the needs of families with newborn children. The CAS will solicit feedback from home visitors and supervisors regarding unmet needs in the community and, with the team, plan toward closing those gaps as possible. In addition, the CAS will lead the marketing and public relations efforts of the Family Connects NM services. The CAS will also participate in establishing and maintaining communication between community resources, physicians, nursing personnel and other health-care professionals regarding service objectives, policies and procedures. The CAS will compile and maintain records, reports and documentation of service activities regarding community relations for use in service evaluation. In addition the CAS will strengthen community support for families with newborns. Having a newborn is a universal time of need; the variable is the amount of need each family has. The FCNM team continuously works to de-stigmatize home visiting, a notion that is critical to the success of a universal approach in supporting all families with a newborn.

How does it get done?

1. Establish and nurture relationships, partnerships, and coalitions with community resources and services that provide support to families in a timely and appropriate manner as referred by Family Connects New Mexico nurses and that participate in the community system of care for young children and their families.
2. Identify resources, establish relationships with providers, maintain an updated resource portal, and identify gaps in needed community resources so that the broader community can be induced to grow these resources over time.
3. Attend nurse case conference meeting to provide individual-case consultation about community resources to support nurse home visitors as they connect families with community resources, as needed.

5. Market the services by communicating with referral sources (for example, OB/GYN practitioners, hospitals, pediatricians),communicating with groups that interact with families (for example, employers, newspapers, churches, childcare agencies), and soliciting support from community leaders (for example, to publicize the services and to provide incentives for families.
6. Increase the array of options for pregnant women and new mothers to know about and to sign up for a Family Connects home visit by work with health providers and other organizations about referrals to and support for the service.
7. Organize a community advisory board for Family Connects New Mexico service as it coordinates services with others in the early childhood system of care.
8. Compile and maintain records, reports, and documentation of service activities regarding community relations for use in service evaluation.

All caregivers, newborns and community partners that work to support families.

Ideal Candidate

1. Master’s degree in a related field (for example, public health, psychology, social work, nursing communications, marketing), or a Bachelor’s degree with additional related experience.
2. Requires progressive experience in human services in the public sector, skill in interacting with human service providers and agency leaders, and an understanding of the role of evaluation in social services.
3. A combined five years of experience in a related field is desired.

Minimum Qualification

Bachelor’s degree in Public Health or Education and two (2) years of experience in a health related field. Substitutions Apply. See Substitution Table below.

Employment Requirements

Must possess and maintain a valid Driver’s License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment.

Working Conditions

Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. Some travel may be required.

Services are provided in communities, clinics, axillary site settings ,and in the homes of newborns and their care givers. Reaching and lifting up to 50lbs may be required. Travel to conduct community partner meetings with the occasional overnight stay required (less than 5 times per year).
