Description

East End Community Services is a 25-year old non-profit organization whose mission is to break the cycle of generational poverty in Dayton Ohio. Through the process of skilled coaching we help empower disadvantaged individuals and whole families to explore the change process, set goals, and create specific plans for building new skills and self-confidence to achieve overall success, thereby disrupting the cycle of poverty.

We offer programs in education (out-of-school-time academic, social and emotional support for K-8 students and high school age youth; plus parent education), employment and income stability (credential based job skills training, computer training, job related soft skills training, job placement), health services (support for struggling with addiction recovery), access to affordable housing and healthy food, and social connectedness (opportunities to build supportive relationships with others).

Position Summary:

The Community Health Coordinator provides health care coordination services to Medicaid-eligible persons. Initially, this role will work with pregnant mothers to help them improve health outcomes for themselves and their families. Job Functions include those listed below.

Essential Job Functions:

  • Recruits and enrolls Medicaid-eligible persons to the Pathways HUB program;
  • Canvasses clinics, community centers, emergency departments, and other community sites to find individuals eligible for enrollment into the Pathways HUB;
  • Contacts prospective clients within 48 hours of receiving a referral;
  • Enrolls all eligible clients who are uninsured into Medicaid (Health Insurance Pathway);
  • Ensures all clients without a primary care provider are connected to a medical home within 30 days of program enrollment (Medical Home Pathway);
  • Ensures clients schedule and attend medical appointments with any providers as needed (Medical Referral Pathway);
  • Works with enrolled clients to identify needs that impact the health of the enrollee and connects them to community-based resources and services ;
  • Maintains a full-time active caseload of approximately 30 active and enrolled clients after agreed upon ramp up period;
  • Maintains complete, accurate, and timely documentation of services provided;
  • Opens and completes any other Pathways specific to the population being served that address the social determinants of health;
  • Attends 75%+ of all scheduled mandatory trainings and conference calls as required by the Pathways HUB.
  • Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation;
  • Performs any other duties as assigned.

Qualifications

Certifications/Licensures:

  • OBN or relevant certification and/or licensure ispreferred; preference will be given to certified candidates;
  • Valid drivers license and the ability to obtain and maintain WestCares vehicle insurance is required.

Education:

  • Graduation from an accredited two-year or college with a degree in Health Services, Social Work, or other field related to the position is required;
  • High School Diploma or equivalent is required.

Experience and Competencies:

  • At least four (4) years of experience providing case management services focused on health care access, and lived experience working in low-opportunity communities;
  • Ability to establish priorities, work independently, and proceed with objectives required;
  • Ability to handle and resolve recurring problems required;
  • Relates extremely well with people, particularly disadvantaged populations required;
  • Experience effectively managing a caseload required;
  • Ability to develop meaningful goals and objectives required;
  • High level of personal accountability for the quality and impact of work required;
  • Demonstrated ability to exercise sound judgement required;
  • Excellent verbal and written communication skills required;
  • Ability to form and maintain effective, professional, and positive relationships required;
  • Must be empathetic, compassionate, non-judgmental, and enthusiastic;
