The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The Colorado Coalition for the Homeless seeks a Community Reintegration Specialist, Fort Lyon to provide transition planning and community reintegration for adults who are homeless – including those with disabilities – suffering from drug and alcohol addiction, mental illness, and seeking transitional and permanent housing. This position works directly with Residents and in support of Case Management to help people meet needs related to health, mental health, substance abuse, employment, housing and benefits. The Community Reintegration Specialist/Driver is central in ensuring that Residents are sufficiently prepared to transition into their community of choice after completing their Fort Lyon Program goals; requirement to provide local transportation in conjunction with local community reintegration.This position is located at 30999 County Road 15., Las Animas, Colorado 81054and reports to the Associate Director.

Essential Job Functions
- Facilitates outreach and engagement with former Fort Lyon residents to assist in addressing their community-based needs.
- Provides Denver/Fort Lyon transportation including new residents, discharges and residents going on pass usually two times per week.
- Coordinates with discharge planner and resource team in advance of residents planned transition to housing.
- Identifies and engages community employment support such as Vocational Rehabilitation, VA and State/County Workforce.
- Works with local providers and referral sources to determine what community resources are available in their area.
- Engages with alumni through Friends of Ft Lyon meetings for locally based graduates.
- Leads the Resource Team and evaluate residents’ reintegration needs, to include transportation, ongoing medical/mental health treatment, furniture, household goods, food.
- Assists with physical moves of Residents to new housing.
- Completes all required service tracking documentation in a timely manner.
- Participates in team and supervisory meetings as assigned.
- Meet required COVID-19 vaccination(s), as defined by applicable law, regulation, or guidance from the Centers for Medicare and Medicaid Services.

Qualifications Summary
- Bachelor’s degree in social work, counseling, or similar field preferred.
- Experience and interest in working with disenfranchised persons.
- Must have a valid driver’s license and ability to transport clients in personal automobile.
