Social Case Worker I, Intake Focused

PURPOSE OF POSITION:
An Intake Social Case Worker at Catholic Charities:
- Follows procedures and guidelines set forth in operating guidelines manual(s) to help maintain structure and order in the living environment.
- Manages all pre-screens and referrals for the Veterans and Client Services team.
- Assigns new clients to SCW team for case management.
- Complete VI-SPDAT’s.
- Reviews referrals from community partners and accesses readiness for Samaritan House program.
- Accurately and promptly completes all required documentation/data, including entering relevant participant information in daily logs before ending shift assignment.
- Manages waitlist for Veterans and Client Services, including follow up communication, directing waitlist questions, etc.
- Enforces shelter rules uniformly and consistently
- Communicates with other staff to resolve problems with individual participants
- Assists in the development and implementation of case management services that helps participants become more independent and functional.
- Conducts in-take and orientation of new participants, including providing tours to new intakes.
- Networks within the community resources to achieve the best services available for the participants.
- Advocates with the participants when they have barriers to overcome in meeting their goals.
- Develops and maintains appropriate records, statistics and documentation of services to the participants.
- Maintains confidentiality of participant and agency information.
- Spends time at Women’s Emergency Shelters to complete referrals and support staff in training.
- Uses creativity and innovation in program development and service delivery.
- Completes HMIS 101 and HMIS 201 training and accurately enters and maintains HMIS enrollments.
- Regular and predictable attendance.
Qualifications
QUALIFICATIONS
- Ability to understand the challenges faced by special need populations in the community.
- Capacity to identify participant barriers to activities of daily living.
- Ability to use computer to enter participant’s information into case files and compile program statistics, fax machine, copier, telephone and motor vehicle.
- Aptitude to assist participant in identified barriers.
- Knowledge of the service population’s cultural and socioeconomic characteristics and the appropriate techniques and practices for the participant population.
- Ability to interact effectively as a team member and independently with Agency staff and with a diverse participant base.
EDUCATION and/or EXPERIENCE
Bachelor’s degree (BA/BS) from accredited four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
COMPENSATION & BENEFITS:
Salary: $20.00/hr plus $1/hr bilingual differential for Spanish
Schedule: This is a full-time position, 40 hours per week. Some flexibility, but the shelter is open 24/7 and will work on schedule to fit business need, including some evenings.
