Description

ESSENTIAL DUTIES AND RESPONSIBILITIES

Under the general supervision of the Director, the Coordinator of Home Delivered Meals is responsible for maintaining contact with clients who receive home-delivered meals and the meal delivery team to ensure that services are coordinated with case management/coordination staff and meals are delivered as scheduled. Specific duties and responsibilities include:

  • Receive and respond to incoming calls from clients and delivery staff; record call data into the funding source database
  • Develop and maintain home-delivered meal route sheets
  • Report and coordinate with the case management staff regarding daily delivery and coordination of client services
  • Maintain client files and document all client contact, case coordination, service changes, or contact with case management regarding client status, change in service, or client concerns
  • Verify delivery route information uploaded to the funding source database; enter route sheet data as needed into the database when information is not automatically uploaded via the mobile app
  • Review all route sheets for proper documentation of delivery, unresponsive clients, return date & time, and when necessary, the required employee signature
  • Perform an inspection of all delivery trucks upon return from assigned routes to ensure proper maintenance and for safety purposes
  • Coordinate with the drivers and deliverers in the field on best practices handle emergencies and/or issues as they arise; report any serious emergencies or staffing needs to the Program Director or Assistant Director
  • Prepare and/or account for charitable donation receipts and letters with the program bookkeeper or designated staff person for audit purposes
  • Promote and represent RiseBoro in the community; attend interagency events, activities, and other recommended networking opportunities
  • Other related duties as required by the Director

EDUCATION EXPERIENCE

  • Associate degree with a minimum of one (1) year of social service, customer service, or related experience preferred
  • High School Diploma or equivalent with a minimum of two (2) years of social service, customer service, or related experience
