Description

Summary: Provides triage, crisis counseling and linkage on-demand in any setting in which a crisis is occurring including homes, schools, or community based setting within 60 minute on-site response time

Illustrative Duties and Performance Responsibilities:

  1. Works with individuals, families, and groups to provide outreach, emotional support, individual and group crisis counseling
  2. Provides warm hand off to community resources, services and supports
  3. Triages phone calls and provides community resource information as well as supportive counseling
  4. Conducts screening & assessments and provides intervention services
  5. Engages individuals and families in the crisis planning process using evidenced based practices including Motivational Interviewing and Mental Health First Aid.
  6. Develops safety or crisis care plan
  7. Facilitates stabilization and care coordination services post acute intervention
  8. Coordinates psychiatric face to face or telehealth appointments with psychiatrist within 48 hours of intervention
  9. Provides after hours crisis services on a rotation schedule
  10. Interacts with management to gather and provide information

Qualifications

Minimum Qualifications:

  • Education: Bachelors Level Mental Health Professional with degree in Human Services.
  • Language Skills:
    • Ability to read, analyze, and interpret agency and grant required documents
    • Ability to respond effectively to the most sensitive inquiries or complaints
  • Experience:
    • Training and or Experience in Crisis Intervention Preferred
    • Familiarity with Monroe county
  • Reasoning Ability:
    • Ability to collect data, establish facts, and draw valid conclusions.
    • Understands, communicates (both verbally and in writing), and collaborates effectively and with precision, given the time constraints of each task.
  • Other Skills and Abilities:
    • Ability to provide an Affidavit of Good Moral Character
    • Must have reliable transportation to travel throughout the county
    • Clearance in PreEmployment Drug Testing and Criminal Records Checks, as required.
    • Possesses sufficient means of independent mobility.
