Summary

The Community Case Manager (CCM) is a part of the Drop-In mental health and social services program. These services include resource navigation support, referrals, crisis de-escalation, socialization, community building, and support groups. The CCM works in a fast-paced environment that seeks to serve all veterans and is comfortable with managing a wide variety of tasks. The CCM is experienced and comfortable engaging with veterans who may be intoxicated and/or presenting with active mental health symptoms.

Essential Responsibilities include but not limited to:
• Engages with veterans to screen and triage access to services at Swords to Plowshares.
• Provides a safe and supportive environment for veterans seeking services with an understanding of cultural humility, harm reduction, and trauma informed care practices.
• Completes general intake into Swords to Plowshares services and makes interagency program referrals for all eligible veterans and follow-up when necessary.
• Assists veterans with resource navigation support and linkage to local community and veterans’ social services.
• Enters veteran intake data into our electronic database (ECM), compiles charts, and maintains documentation requirements that follow agency and various funders policies, procedures, and standards.
• Ensures quality of services follow Swords to Plowshares and the various funders policies, procedures, and standards.
• Facilitates 1 veteran client group per week.
• Adheres to laws regarding confidentiality and reporting requirements.
• Performs other duties as assigned by Clinical Director and/or Program Manager.

Education
• High School diploma or GED required.
• BA/BS desired but equivalent professional experience is acceptable.

Experience
• Minimum of one (1) year experience working with complex behavioral health and substance use challenges in a social services fast-paced office setting.
• Translatable military experience and skills may be alternatively substituted.

Skills
• Strong written, communication and listening skills.
• Excellent Computer literacy, with proficiency in Microsoft Office suite.
• Valid California Driver’s license, clean driving record. Must be able to drive the company van.
• Organized with attention to detail.
• Ability to follow through with tasks in a timely manner.
• Efficient and accurate data entry and documentation in ECM (Exponent Case Management) electronic database and in client files.
• Ability to manage time efficiently.
• Ability to set healthy boundaries and acknowledge self-care needs
• Ability to communicate with coworkers and supervisor/s in a healthy and professional manner
• Ability to appropriately respond to conflict and/or emergency situations in support of resolution.
• Working knowledge of trauma informed care practices and harm reduction model
• Ability to work independently and as a team player.
• Must be able to communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agency’s Diversity, Equity, and Inclusion Initiative.

Salary & Benefits

Wage range: This is a Full Time, Non-Exempt position with a starting wage range of $24.50 to – $26.00 per hour

Compensation is based on a competitive public interest salary scale. Swords to Plowshares offers a generous benefits package including the following benefits:
