Family Habilitation Specialist
Compensation: $18.00 per hour
Position Summary:
The Family Habilitation Specialist works to make a meaningful difference in the lives of the DCFS involved families being served by the National Youth Advocate Program by working closely with the parents and other caregivers to provide skills training, advocacy and support towards targeted treatment goals.

Responsibilites:
The Family Habilitation Specialist will perform duties including, but not limited to:
- Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.
- Assist parents or other caregivers to maintain or reunify their family unit through protective services that in maintaining the family and strengthening the ability of parents or caregivers to provide adequate childcare and to improve parenting skills.
- Provide in-home supportive services, parenting education, counseling, community resource referral, advocacy, and other social service interventions to the youth and family.
- Assist in family reunification and preservation efforts with all members of the primary family or other permanent plans consistent with the service plan.
- Work collaboratively with DCFS Caseworkers and Supervisors to ensure successful outcomes for families
- Contact the assigned families within 24 hours of assignment.
- Contact DCFS caseworkers and supervisors for an initial meeting within 48 hours of assignment.
- Obtain a copy of the DCFS Service Plan and service objectives from the assigned DCFS worker and supervisor.
- May be may be required to provide services for 24 hours intervals for short periods of time to ensure safety and prevent out of home placement during a family crisis. (not to exceed 72 hours)
- Complete case notes and reports weekly for submission to DCFS caseworkers and supervisors.
- Complete all required documentation with the parents per DCFS contract terms.
- Meet with the youth and family at least weekly to review the progress toward treatment goals.
- Coordinate and monitor services for the parent/caregiver in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services.
- Participate in administrative and court reviews of the case plan as requested by DCFS
- Complete incident reports and all other required reports according to agency policy.
- Attend weekly supervision meetings and actively participate in the supervision process.
- Follow agency procedures for after-hours and weekend emergency coverage.
- Serve as a positive and effective liaison between NYAP and referring agency caseworkers regarding the youth and family services.
Minimum Qualifications:
- Must have a minimum of a High school Diploma or GED
- Must be 21 years of age or older
- Two years’ experience in youth services, preferably with direct experience in child welfare services.
Other Requirements:
- Demonstrate a sincere commitment and ability to advocate for youth and families facing mental health, education, developmental, and socioeconomic challenges.
- Reliable transportation and a valid driver’s license
- A minimum of $100,000/$300,000 liability insurance on vehicle used for transportation
- Works well independently and as a team member
- Multi-task efficiently and be flexible in all situations
- Openness to traveling as required
