Responsibilities:

The Patient Navigator oversees a caseload of outreach patients and acts as the first point of contact between the patient and the program to which they are assigned. The Patient Navigator is reponsible for locating Health Home candidates received from our Lead Health Homes, as well as from community referrals. The Navigator is to use various outreach mediums to locate potential clients as well as explaining the benefits of the program and obtaining agreements and consents for enrollment.

Qualifications:

  • Associate’s Degree preferred
  • 1-2 years minimum experience in case management and/or care coordinations
  • Fluency in Spanish required.
  • Ability to successfuly engage clients and build relationships with staff and providers.
