The role of the Health Unit Coordinator is to assist the Unit Manager and the nursing staff / interdisciplinary team in the day-to-day communications and the clerical tasks and organization of the nursing unit. All assigned tasks are carried out in accordance with policy and procedures of Fellowship Manor and the Mission and RISE Core Values of Fellowship Community.

ESSENTIALQUALIFICATIONS/CREDENTIALS

§ Highschool education required.

§ Post-secondaryeducation preferred.

§ Proficiencyin medical terminology required.

§ Must beable to read, write, comprehend and speak English.

§ Intermediate/advancedcomputer skills (Email, Microsoft Word, MatrixCare) required.

§ Must demonstrateeffective communication and people skills.

§ Must becertified in Basic Life Support.

§ Mustunderstand geriatrics, dementia short-term and long-term care.

§ Clericalexperience is preferred.

§ Abilityto prioritize, organize and address demanding needs as they arise.

§ Mustpossess positive customer service skills to address internal and externalcustomers.

§ Workperformance must reflect the essential elements of Fellowship Community’s CoreValues.

§ Mustsuccessfully complete the new hire Fellowship Community orientation.

§ Meetsthe requirements of the pre-hire assessment.

ESSENTIAL DUTIES ANDRESPONSIBILITIES:

Maintain active medical records in a neat organized fashion (includes filing).

§ Thinactive medical records as directed.

§ UsesMatrixCare software competently.

§ Orderand maintain adequate levels of supplies for nursing units.

§ Recordmedical and administrative information in accordance with established chartingand documentation, policies and procedures.

§ Maintainup-to-date resident census for assigned unit.

§ Copying/scanning of documents as assigned.

§ E-mailand fax communications as necessary.

§ Answertelephone, page calls, deliver messages to residents, employees,

etc.,as necessary.

§ Maintaina current listing of up-to-date phone numbers.

§ Recordappropriate resident identification data on designated medical

records, MatrixCare, wristbands, and picture ID for emergency box andMAR.

§ Recordvital signs, I & O’s and weights daily.

§ Forwarddiet changes to the dietary department.

§ Keepnursing services personnel informed of resident’s appointments.

§ Assistin arranging and making resident appointments for diagnostic

and therapeutic services and processing thepaperwork.

§ Arrangefor Fellowship Manor personnel to transport residents as necessary.

§ Arrange/scheduletransports as necessary outside the facility.

§ Assistwith all admissions, transfers and discharges as directed by the

Clinical Care Manager /Case Manager.

§ Assistin scheduling of personnel and staffing as directed by Staffing Coordinator/ClinicalCare Manager/Supervisor.

§ Informnursing personnel of new admissions, expected time of arrival

and room assignments. Prepare chart as per protocol and assure roomis ready.

§ Restockthe form drawers as necessary.

  • Keep nurse stations organized, neat and stocked in a uniform manner.
  • Provides educational information to the resident’s family as appropriate.

§ Greetnewly admitted resident/family members and escort to assigned room.

§ Informfamily members of resident needs, occurrences, appointments, etc.,

as directed by Nurse Manager/Team Leader.

§ Assistin transport of residents to and from activities as required.

§ Assistin orientation of new employees as necessary.

§ Treatall residents with dignity and respect.

§ Reportall instances or allegations of resident abuse.

§ Keepresident information in the emergency box up-to-date.

§ Assistin the MDS process as directed.

§ AssistMedical Records in the closing of a resident chart.

§ Supporta restraint-free environment and resident safety.
