HIM Document Imaging Specialist

Overview

The position is responsible for organizing and maintaining health data in electronic, paper systems as well as scanning and indexing all paper and faxed documents into the Electronic Health Record System(EHRS) with the highest possible
image quality. Verifies that all documents are indexed to the appropriate patient chart and document category. They are responsible for reviewing patient records, organizing databases, tracking patient outcomes and protecting patients’ health information.

**** This position is considered a hybrid role. The chosen candidate will be reporting to headquarters in Austin, Texas. This position is open to all applicants in the greater Central Texas region. ****

Salary: $17-18 dollars an hour DOE

Responsibilities

Verifies appropriate patient identification on each document.
Identifies documents to be maintained or discarded prior to scanning.
Prepares paper documents for efficient scanning by fixing torn edges, removing staples.
Scans documents into the electronic document imaging system.
Reviews each image for quality.
Performs rescans as necessary.
Determine classification of documents in correct document type categories and patient medical record.
Reroutes documents needing wet signature to providers.
Responsible for accurate and timely record processing.
Performs daily maintenance of scanners to ensure image quality.
Meets set productivity standards.
Answers phone calls and responds to inquiries for assistance.
Ensure all tasks provided and associated with patient care, patient administrative processes and related duties
comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare
Standard Operating Procedures and CommUnityCare Policies and Procedures.
Provide a high level of customer service to all employees within the organization.
Establish and ensure favorable relationships throughout the organization.

Maintain confidentiality of sensitive information and data.

Care Collaborative, co-workers, including clinical managers, clinical support staff, providers and business office
staff and other partners.
Performs other duties as assigned.

PRIMARY ACCOUNTABILITIES

Prepares new patient charts, gathering documents and information from paper sources and/or electronic health
record.
Ensures medical records are assembled in standard order and are accurate and complete.
Creates digital images of paperwork to be stored in the electronic medical record.
Files paperwork and reports in patient charts, ensuring they are completed in an accurate and timely manner.
Ensures files are stored in the designated area according to storage procedures.
Responsible for safeguarding patient records and ensuring compliance with HIPAA standards.
Responds to requests for patient records, both within the facility and by external sources, retrieving them and
transmitting them appropriately.
Retrieves medical records from offsite chart storage facilities when needed.
Performs other clerical tasks as needed, such as answering phones, faxing, and patient check in and out.

Qualifications

Education

  • High school diploma or equivalent.

Knowledge and Experience

  • Computer skills, including e-mail, word processing, scanning.
