THE POSITION:

The Systems Matcher, reporting to the Director of Regional Coordination, oversees the process of matching permanent housing resources for homeless adults in Service Planning Area (SPA) 2. This position serve as a liaison to partner agencies responsible for placing homeless individuals in the agency’s Bridge Housing programs, ensuring continuous provision of housing services. This role involves extensive coordination with external housing systems, community agencies, and housing providers, as well as collaboration with various departments within LA Family Housing, such as programs staff and Property Management. The Systems Matcher plays a crucial role in optimizing outcomes for individuals transitioning into permanent housing, requiring a combination of creativity, organization, and independent judgment.

WHAT YOU’LL DO:

MATCHING CENTRALIZED HOUSING RESOURCES

  • Uses Homeless Management Information Systems (HMIS) to prioritize individuals who are most appropriate for the centralized housing resources
  • Works closely with local permanent supportive housing providers, as well as the Department of Mental Health and the Department of Health Services, to ensure different housing pathway criteria are met
  • Tracks and manages the matching process, both through HMIS and through internal CES tracking systems
  • Serves as SPA 2 representative in county-wide efforts to develop future data platform utilization that supports the matching process
  • Leads bi-monthly regional case conference meetings to ensure prioritized individuals are matched to housing and programs
  • Attends bi-monthly team meetings with LAHSA to stay up to date and be an active participant in the creation of new CES policies
  • Initiates warm handoff between case manager and provider for matched participants, as well as create public alert on Clarity to notify all agencies of participants’ matches
  • Monitors and follow-up with all active matches to resolve issues and ensure all parties are aware of match

HOUSING LIAISON

  • Develops and maintains relationships with outside partner agencies who utilize the Coordinated Entry System in SPA 2
  • Continuously educate outside partner agencies, as well as LAFH staff, with the most up-to-date information regarding CES policy and available resources
  • Acts as a liaison between case managers and LAHSA, HACLA, & HACoLA to ensure SPA 2 participants and case managers have an advocate within CES

REPORTING ACTIVITIES

  • Submits all tracking logs and reports requested by LAHSA
  • Documents new matcher responsibilities as created by LAHSA in the SPA 2 Adult & Family Matching Manual
  • Reviews data reports prior to submission, and work with appropriate staff/agencies to correct inaccuracies or data errors
  • Drives personal vehicle in and around Los Angeles County and drive agency vehicles periodically to transport clients

OTHER

  • Additional tasks, projects, and responsibilities as assigned by supervisor

WHAT YOU’RE SKILLED AT:

  • Proven work experience as a trainer, training facilitator, and/or project manager managing multiple training events in a corporate setting
  • Proven ability to complete full training cycle (assess needs, plan, develop, coordinate, monitor, and evaluate)
  • Familiarity with the methods and principles for designing programs to teach, train and instruct both groups and individuals, including the means of assessing the effects of training
  • Familiar with working in a multicultural environment and accustomed to working with people from various backgrounds.
  • Strong written, verbal, presentation, and communication skills; ability to communicate clearly
  • Strong administrative and project management skills

  • Should possess strong attention to detail and solid organizational skills, with pride in accuracy and quality of work; able to be highly collaborative within a solid team environment
  • Ability to work in a fast-paced environment
  • Strong decision-making skills
  • Strong skills in database management and record keeping
  • Proficient in Microsoft Programs (Word, Excel, Access, PowerPoint) and HMIS
  • Bilingual (English/Spanish) speaking and writing skills are strongly preferred
