Position Overview
The Illinois Department of Children and Family Services is seeking an organized, professional, and results oriented individual to serve as Independent Living/Transitional Living Monitor. Under general supervision, this position will serve as monitor of private agencies that provide implementation of Independent Living Operations/Transitional Living Programs (ILO/TLP). The position will drive to on-site locations to provide consultation and direction to private agency staff regarding ILO/TLP for the Department of Children and Family Services. The position monitors to ensure that private agencies are in compliance with DCFS rules, policies, and mandates and reviews selected client files on a monthly basis. The position will also work with ILO/TLP agencies to develop systems to address problematic issues. This position provides a great opportunity for someone who is focused on monitoring to ensure that youth in care successfully move to independent or transitional living. Job Responsibilities
1. Serves as Independent Living/Transitional Living Monitor.
2..Works with ILO/TLP agencies to develop systems to address problematic issues.
3. Develops, maintains, and distributes to appropriate purchase of service (POS) and DCFS staff profiles of all ILO/TLP providers
4. Prepares necessary documents for evaluating agency services
5. Provides assistance in the scheduling and coordination of quarterly meetings to provide needed information and resources to providers. .
6.Reviews CYCIS data to ensure accuracy of information regarding client living arrangements.
7. Organizes monthly ILO/TLP meetings
8. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
Preferably requires a master’s degree in social work.
or
Requires a master’s degree in a related human service field, supplemented by one (1) year of directly related professional casework/case management experience.
or
Requires a bachelor’s degree in social work and one (1) year of directly related professional casework/case management experience.
or
Requires a bachelor’s degree in a related human service field and two (2) years of directly related professional casework/case management experience.
The college or university issuing a bachelor’s or master’s degree must be accredited, and the degree program in social work must be approved by the Council on Social Work Education. The directly related professional casework/case management experience must be related to family preservation, family reunification, adoption, youth development, counseling, and advocacy services or a related field.
