Description

Critical Response Engagement Worker III

(Clinical and Responsive Engagement Services)

C.A.R.E.S workers provide short term therapy to children and families that are involved with the child welfare system, targeting youth ages 7-18 presenting with mild to high clinical/therapeutic need necessary for immediate intervention with various case complexities on a short-term basis.

EDUCATION

Master’s Level Clinician; Social Work or Related Field

License Required: LCSW, LPC, LMFT

EXPERIENCE

  • Experience with trauma informed care.
  • Experience with individual & group therapy required.
  • A minimum of one year of experience working with children & families within the clinical realm.

QUALIFICATIONS

  • Strong clinical skills and knowledge of the treatment issues concerning children, adolescents, adults, and families
  • Must have experience in crisis prevention and intervention service.
  • Must possess the ability to work cooperatively with direct care staff.
  • Must be able to intervene in emergency situations.
  • Must have experience in working with families and staff of various backgrounds with a primary focus on families of African American heritage.
  • Experience in developing training curriculum for staff and foster parent enrichment strongly preferred.

RESPONSIBILITIES

  • Possess the knowledge of kinship foster care programs and client/family services.
  • Maintain a caseload of 10-12 youth (~24 sessions)
  • Upon case assignment, the Critical Response Engagement Worker will conduct Clinical Intake Assessment then develop and implement a treatment plan.
  • The Critical Response Engagement Worker will maintain records including contact log, encounter forms, treatment plan, case notes, and discharge summary. The Critical Response Engagement Worker will work with C.A.R.E.S Director and collaborate with additional service providers to ensure appropriate service delivery if more intensive services are necessary.
  • Serve high-risk/high need kinship families, including those whose children are at substantial risk of placement disruption.
  • Manage and maintain all clinical documentation, activities, sessions, etc., to remain in compliance with state regulations and license.
  • Provides therapy sessions to children and families in home, community, and agency settings based upon assessed need.
  • Demonstrate proven sensitivity to the various cultural and socioeconomic characteristics of the identified children and families referred to the agency
  • Conduct critical case management service and consultation within various settings with the kinship triad to identify challenges and behaviors of concern that are potentially impeding healing and overall well-being of children and families.
  • Must work from a strength-based perspective.
  • Complete assessment instruments that assist the kinship family to recognize strengths that promote individual well-being, family preservation and enhance family functioning.
  • Develop and implement critical case management plans and interventions for children and families within the ~8 week C.A.R.E.S service model.
  • Refer the family to community based mental health services if specialized or intensive mental health services are necessary
  • Dedicate time to weekly case planning and evaluation of case progress, with continued support through supervision and consistent coaching.
  • Collaborates and maintains professional communication with internal and external support staff including but not limited to POC, HIU, and other partnering agencies.
  • Utilize results from the YSA, Clinical Intake and HIU Intake meeting, the Critical Response Engagement Workerwill develop and implement the Clinical Treatment Plan, collaborate with HIU Liaison and subsequent support staff to ensure well-rounded service delivery.
  • Maintain records including contact log, encounter forms, treatment plan, case notes, and discharge summary.
  • Works with C.A.R.E.S Director to monitor progress towards discharge, participates in HIU meetings to support wrapped services, maintains regular communication with additional service providers to align supports, and develops a step-down plan that may include Level 3 referral for continued support post-HIU.
  • Engage in regular training, professional development, supervision, and guided activities designed to help build extensive clinical skills
  • Act as a consultant in case reviews with ASCI case management staff facilitating Critical Case Conferences and quality review meetings
  • Participate in all clinical departmental goals and tasks (example: mental health awareness campaign, mental health training development, professional development etc.).
  • Responsible for participating in an on call rotation in addition to a non-traditional work schedule defined by family needs.
  • Other duties as assigned.

  • Licensed professional in the State of Pennsylvania preferred

  • Current Act 33, Police Clearances and FBI Clearances
  • City/County travel
  • Medical examination and TB screening

This position reports directly to the Director of In Home Clinical Services.
