Description
The Director of Social Services provides Social Services to Residents, their families, and significant others to enable them to deal with the impact of illness and extended care placement and to preserve and enhance the highest practical physical, mental, and psychosocial well-being.
Essential Functions, Roles, and Responsibilities
- Managed and implements social service plans, policies and procedures.
- Participation in resident care management. Care management will include the following:
- Facilitates and participates in regularly scheduled care plan conferences.
- Assist in the development and ongoing review/revision of multi-disciplinary plan of care.
- Obtain information pertinent to resident evaluation and development of care plan, including but not limited to, cognitive, behavior, depression, and physical restraint assessments, review of documentation/flow sheets of other departments, physicians, consultants, agencies, and attending unit reports.
- Meeting personal and psychosocial needs of residents, including but not limited to, SS referrals, co-ordination of room changes, and changes in Resuscitation choice or advanced directives.
- Coordinating with Residents, family, staff and community resources in meeting the needs of residents.
- Coordinating services with other disciplines within the facility to enable the residents and families to promote their own well-being.
- Provide supportive visits, crisis intervention, or group services with Residents and/or families when appropriate or requested.
- Perform timely documentation of Social Service interventions and assessments in Resident’s chart to include interviews for the MDS, social histories, recaps or discharges.
- Provide an avenue for family and resident complaints and procedures for resolutions.
- Assure that Residents and their Legal Representatives are informed of their rights upon admission and at least annually thereafter and that the rights are maintained and protected.
- Complete level of care/PASRR screenings.
- Hospice referrals and set up.
- Responsible for performing Admissions and Coordinating Discharges.
- Conduct investigations of alleged abuse or neglect of Residents as per Facility policy.
- Conduct inservice training for staff as needed.
Requirements
Must be a Licensed Social Worker or Social Services Designee that has a passion for advocating for resident’s best interest. Must communicate effectively to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public. Respects the dignity and privacy of all residents and any confidential information.
