Job Title: Licensed Practical Nurse (LPN Supervisor)
Department: Crisis Support Services
Reports To: Director of Clinical and Crisis Support Services
Supervises: Direct Support Professional, License Practical Nurse
FLSA Status: Non Exempt
SUMMARY
The primary function of the Licensed Practical Nurse is performing nursing activities, supports, care and treatment to children and adolescents in accordance with the policies of Georgia Community Support and Solutions, Inc (GCSS) dba InCommunity and the State of Georgia Nursing Licensing Bureau. The Licensed Practical Nurse will be expected to work closely with an interdisciplinary team (BCBA, Psychiatrist, Psychologist, etc), related service providers, families and other agencies as appropriate.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Responsible for the day to day management of the unit
- Supervise the administration of medication and complete health assessment
- Review all admission referrals for appropriateness of services
- Perform medication management functions
- Conducts assessments and evaluations as needed
- Direct Supervisor of LPNs
- Ensures that the crisis homes are adequately staffed
- On call for staffing the crisis homes as the need arises for such coverage including contacting agency or other staff in order to ensure that the home is adequately staffed
- Psychiatry Review- Contacts doctor as needed to report, clarify or discuss issues.
- Medication- Review doctors orders, review and sign off on LPN notes, review Medication Adminstration Records (MAR), change MAR as needed. Assist staff with medication issues.
- Weekly review and audit of LPN records to ensure compliance
- Monitor house funds, i.e. making funds available for recreational events, document individual spending of funds, collect receipts and follow GCSS administrative finance procedures and policies
- Develop weekly/monthly menu with individual assistance;
- Facilitate and/or participate in staff meetings of crisis home staff .
- Identify and setup educational/training needs of staff
- Provide Health related education trainings to staff to meet the individuals needs
- Meet with the Director of Nursing on a routine basis to discuss and resolve current issues.
- Serve on CQI committee to ensure areas that need improvement are identified and in collaboration with other members of Crisis Staff, develop and implement measures to promote improvement.
- Serve as a liaison when the home requires nursing consultation to meet the needs of individuals
- Assists with the interviewing, hiring and training employees for the crisis homes.
- Assist with disciplinary actions and completes payroll duties
- Complete performance appraisal of subordinate staff
- Review nursing progress notes and discuss with the appropriate staff member any discrepancies notes on the record (i.e. lack of documentation, inappropriate restrictions or lack of appropriate consequences; situations in which the current behavior program or treatment approach were not followed appropriately; check each report to ensure that time, date, signatures are correct).
- Maintain highly organized, consistent, thorough and systematic recordkeeping practices.
- Review and audit nursing records to ensure compliance. Conduct structured periodic service reviews to monitor the effectiveness of the program and its implementation; modify and document plan changes as needed.
- Provide consultation regarding crisis intervention and critical incident supports.
- Complete incident reports within 24 hours as required
- Collaborate with Director of Clinical & Crisis Support Services and Director of Nursing to ensure the homes are managed well and run efficiently and in accordance to all applicable Rules and Regulations.
- Participate in professional growth activities such as conferences, classes, team meetings and program visitations
- Accept all other responsibilities and duties as assigned
EDUCATION/EXPERIENCE
- Graduated from an accredited School of Nursing
- A minimum of at least two(2) years experience serving individuals with developmental disabilities.
- A minimum of three years recent experience working with individuals who have autism spectrum disorders and their families
